How TEFA Funds Apply to Tuition at Forms Academy Residency

If awarded, TEFA funds (estimated at ~$10,474 per standard student for 2026-27, higher for students with disabilities) are placed in a state-managed account. Parents approve direct payments from this account to Forms Academy Residency for tuition and approved fees.

• These payments are credited against your student's tuition balance as received.
• TEFA funds do NOT reduce Forms Academy's published tuition rate or enrollment commitment.
• Families remain fully responsible for the entire annual tuition amount (including any gap between
• TEFA award and total tuition), per the binding financial agreement.
• Disbursements occur in installments throughout the year (e.g., starting July 2026, with additional payments quarterly).
• Coordinate with our office after award notification to ensure smooth application of funds.

RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT

In consideration of being allowed to participate in the event or activity referenced above, I acknowledge, understand, and agree that:

Assumption of Risk – I understand that participation in this program involves significant risks of injury, including but not limited to permanent paralysis, disability, and death. While certain safety measures, including rules, equipment, and personal discipline, may reduce these risks, I acknowledge that the potential for serious injury remains.

Voluntary Acceptance of Risk – I KNOWINGLY AND VOLUNTARILY assume all risks associated with participation, whether known or unknown, including but not limited to risks arising from the NEGLIGENCE of the Releasees or others. I accept full responsibility for my participation and any resulting injury, damage, or loss.

Compliance with Program Rules – I agree to adhere to all stated and customary program rules and conditions for participation. If at any time I observe an unusual or significant hazard, I will immediately cease participation and bring it to the attention of an official.

Comprehensive Release of Liability – On behalf of myself, my heirs, assigns, personal representatives, and next of kin, I HEREBY RELEASE, WAIVE, DISCHARGE, AND HOLD HARMLESS the Releasees, including their officers, directors, agents, employees, sponsors, advertisers, affiliates, volunteers, and, if applicable, owners and lessors of the premises used for the activity (collectively, “RELEASEES”), from any and all liability related to injury, disability, death, or property damage, whether caused by the NEGLIGENCE of the RELEASEES or otherwise, to the fullest extent permitted by law.

Binding Acknowledgment – I acknowledge that:

• I have read and fully understand this Release of Liability and Assumption of Risk Agreement.
• I understand that by signing this agreement, I am waiving substantial legal rights, including my right to sue.
• I am signing this agreement freely and voluntarily, without any inducement or coercion.

US Youth Soccer - Parent/Guardian Consent And Medical Release

Recognizing the possibility of injury or illness, and in consideration for US Youth Soccer and members of US Youth Soccer accepting my son/daughter as a player in the soccer programs and activities of US Youth Soccer and its members (the "Programs"), I consent to my son/daughter participating in the Programs. Further, I hereby release, discharge, and otherwise indemnify US Youth Soccer, its member organizations and sponsors, their employees, associated personnel, and volunteers, including the owner of fields and facilities utilized for the Programs, against any claim by or on behalf of my player son/daughter as a result of my son's/daughter's participation in the Programs and/or being transported to or from the Programs. I hereby authorize the transportation of my son/daughter to or from the Programs.

My player son/daughter has received a physical examination by a licensed medical doctor and has been found physically capable of participating in the sport of soccer. I have provided written notice, which is submitted in conjunction with this release and attached hereto, setting forth any specific issue, condition, or ailment, in addition to what is specified above, that my child has or that may impact my child's participation in the Programs. I give my consent to have an athletic trainer and/or licensed medical doctor or dentist provide my son/daughter with medical assistance and/or treatment and agree to be financially responsible for the reasonable cost of any such assistance and/or treatment.

US Club Soccer Medical Treatment Authorization And Liability Waiver

I hereby give my consent to have an athletic trainer, coach, team manager, emergency medical technician, nurse, medical treatment facility, and/or doctor of medicine or dentistry or associated personnel provide the applicant/participant with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I understand treatment for injury will be based on information provided herein. I hereby authorize emergency transportation of the applicant/participant to a medical treatment facility should an individual listed above consider it to be warranted. I recognize the possibility of physical injury associated with soccer, and hereby release, discharge, and otherwise indemnify the club, US Club Soccer, their sponsors, the USSF and its affiliated organizations, and the employees and associated personnel of these organizations, against any claim by or on behalf of the soccer player named above as a result of that player's participation in US Club Soccer programs and/or being transported to or from the same, which transportation I hereby authorize.

 

Concussion Waiver

This sheet has information to help protect your children or teens from concussion or other serious brain injury. Use this information at your children's or teens' games and practices to learn how to spot a concussion and what to do if a concussion occurs.

What Is a Concussion?

A concussion is a type of traumatic brain injury or TBI caused by a bump, blow, or jolt to the head or by a hit to the body that causes the head and brain to move quickly back and forth. This fast movement can cause the brain to bounce around or twist in the skull, creating chemical changes in the brain and sometimes stretching and damaging the brain cells.

How Can I Help Keep My Children or Teens Safe?

Sports are a great way for children and teens to stay healthy and can help them do well in school. To help lower your children's or teens' chances of getting a concussion or other serious brain injury, you should:

• Help create a culture of safety for the team.
• Work with their coach to teach ways to lower the chances of getting a concussion.
• Talk with your children or teens about concussion and ask if they have concerns about reporting a concussion.
• Talk with them about their concerns; emphasize the importance of reporting concussions and taking time to recover from one.
• Ensure that they follow their coach's rules for safety and the rules of the sport.
• Tell your children or teens that you expect them to practice good sportsmanship at all times.

When appropriate for the sport or activity, teach your children or teens that they must wear a helmet to lower the chances of the most serious types of brain or head injury. However, there is no "concussion-proof" helmet. So, even with a helmet, it is important for children and teens to avoid hits to the head.

How Can I Spot a Possible Concussion?

Children and teens who show or report one or more of the signs and symptoms listed below or simply say they just "don't feel right" after a bump, blow, or jolt to the head or body may have a concussion or other serious brain injury.

Signs Observed by Parents or Coaches:

• Appears dazed or stunned.
• Forgets an instruction, is confused about an assignment or position, or is unsure of the game, score, or opponent.
• Moves clumsily.
• Answer questions slowly.
• Loses consciousness (even briefly).
• Shows mood, behavior, or personality changes.
• Can't recall events prior to or after a hit or fall.

Symptoms Reported by Children and Teens

• Headache or "pressure" in head.
• Nausea or vomiting.
• Balance problems or dizziness, or double or blurry vision.
• Bothered by light or noise.
• Feeling sluggish, hazy, foggy, or groggy.
• Confusion, or concentration or memory problems.
• Just not "feeling right," or "feeling down."

Talk with your children and teens about concussion. Tell them to report their concussion symptoms to you and their coach right away. Some children and teens think concussions aren't serious or worry that if they report a concussion they will lose their position on the team or look weak. Be sure to remind them that it's better to miss one game than the whole season.

Concussions affect each child and teen differently. While most children and teens with a concussion feel better within a couple of weeks, some will have symptoms for months or longer. Talk with your children's or teens' health care provider if their concussion symptoms do not go away or if they get worse after they return to their regular activities.

What Are Some More Serious Danger Signs to Look Out For?

In rare cases, a dangerous collection of blood (hematoma) may form on the brain after a bump, blow, or jolt to the head or body and can squeeze the brain against the skull. Call 9-1-1 or take your child or teen to the emergency department right away if, after a bump, blow, or jolt to the head or body, he or she has one or more of these danger signs:

• One pupil is larger than the other.
• Drowsiness or inability to wake up.
• A headache that gets worse and does not go away.
• Slurred speech, weakness, numbness, or decreased coordination.
• Repeated vomiting or nausea, convulsions or seizures (shaking or twitching).
• Unusual behavior, increased confusion, restlessness, or agitation.
• Loss of consciousness (passed out/knocked out). Even a brief loss of consciousness should be taken seriously.

Children and teens who continue to play while having concussion symptoms or who return to play too soon while the brain is still healing have a greater chance of getting another concussion. A repeat concussion that occurs while the brain is still healing from the first injury can be very serious and can affect a child or teen for a lifetime. It can even be fatal.

What Should I Do If My Child or Teen Has a Possible Concussion? As a parent, if you think your child or teen may have a concussion, you should:

• Remove your child or teen from play.
• Keep your child or teen out of play the day of the injury. Your child or teen should be seen by a health care provider and only return to play with permission from a health care provider who is experienced in evaluating for concussion.
• Ask your child's or teen's health care provider for written instructions on helping your child or teen return to school. You can give the instructions to your child's or teen's school nurse and teacher(s) and return-to-play instructions to the coach and/or athletic trainer.
• Do not try to judge the severity of the injury yourself. Only a health care provider should assess a child or teen for a possible concussion. Concussion signs and symptoms often show up soon after the injury. But you may not know how serious the concussion is at rest, and some symptoms may not show up for hours or days. The brain needs time to heal after a concussion. A child's or teen's return to school and sports should be a gradual process that is carefully managed and monitored by a health care provider.

FORMS ACADEMY PARENT/GUARDIAN CODE OF CONDUCT

As the parent(s) or legal guardian(s) of a student/player in Forms Academy/Forms Academy Residency, we understand, acknowledge, and agree that by enrolling our child in Forms Academy—including any of its programs such as AlphaForms, Alpha Futsal and/or Forms Academy Skills and Technique (FAST)—we are committed to upholding the highest standards of conduct at all times. We agree to comply with the following Parent Code of Conduct:

As Forms Academy Parents, We Will:

• Encourage, support, and affirm our child’s efforts and development on the soccer field, particularly during league and tournament games.
• Respect officials and accept their decisions, recognizing they are integral to the game.
• Respect all facilities, fields, and venue staff at which our child plays and trains.
• Support the coach, manager, and team by fostering a positive and cooperative team environment.
• Complete all required academy forms, including the Consent and Medical Release Form, and volunteer our services when needed to assist our child’s team and the club.
• Familiarize ourselves with the Laws of the Game and the respective facility rules.
• Familiarize ourselves with Forms Academy's methodology in order to better understand the development process each player is undergoing.
• Comply with all team and academy rules, policies, and procedures, including our financial commitment to the club.
• Only discuss our child’s role on the team directly with the coach (not the team manager or any other individual affiliated with the team or academy).
• Discussions with the coach will take place at a mutually agreed-upon time, and never before, during, or immediately after a game.
• Follow the "24-Hour Rule"—waiting a minimum of 24 hours after a game or training session before contacting the coach regarding concerns related to the event.

As Forms Academy Parents, We Will Never:

• Engage in dissent toward coaches, officials, or referees at any time during a game or training session.
• Exhibit unsportsmanlike behavior toward any official, coach, manager, facility employee, player, or parent before, during, or after a game.
• Interfere with the coach’s responsibilities or attempt to instruct players during training sessions, warmups, or matches.
• Act in any manner that is detrimental to the team, the academy, or its programs, including Forms Academy, Alpha Futsal, AlphaForms, and FAST.

Spectator & Sideline Conduct

As a parent spectator, I will watch all games and training sessions from the designated areas.  I understand that coaching, instruction, and game-day decisions are the sole responsibility of the coach. Therefore, I agree to:

• Refrain from coaching from the sidelines during training, warmups, and matches.
• Provide only encouragement while my child is under the instruction of the coach.
• Respect the training and match environments by allowing the coach to manage and instruct without interference.
• By signing below, I acknowledge that I have read, understand, and agree to abide by this Forms Academy Parent Code of Conduct and that failure to do so may result in disciplinary action, including but not limited to warnings, suspension from attending games, or dismissal from the club.

FORMS ACADEMY RESIDENCY EXCLUSIVE COMMITMENT AGREEMENT

As a Forms Academy Residency student-athlete, I acknowledge that my participation in this elite program is a privilege, granted based on my commitment to both the Residency program and the Forms Academy club and school.

I expressly agree that my training, competition, and participation in soccer and futsal games, practices, and events are exclusively for Forms Academy-affiliated teams, including but not limited to Forms Academy, AlphaForms, and Apex. I understand that I am prohibited from training, competing, or representing any other club, team, or organization outside of Forms Academy without prior written approval from Forms Academy leadership.

Furthermore, I recognize that my commitment to the Residency program extends beyond the field. I agree to fully engage in all academic responsibilities, training schedules, and team activities as part of my participation in the program. I understand that my actions, dedication, and adherence to these commitments directly impact my standing in the Residency program.

I acknowledge that failure to comply with this agreement may result in disciplinary action, up to and including removal from the Residency program and Forms Academy.

2026-2027 FORMS ACADEMY RESIDENCY FINANCIAL AGREEMENT

BACKGROUND

I acknowledge that I wish to enroll my child(ren) in Forms Residency (FR). In order for my child(ren) to attend and remain enrolled at FR, I agree to pay in full all tuition, fees, and miscellaneous charges associated with my child(ren)’s enrollment, in accordance with the due dates established by FR. My financial obligation remains in effect until all outstanding debts to FR are fully satisfied.

Failure to pay any outstanding balance by the designated due date may result in penalties outlined in this agreement, including but not limited to disenrollment and restriction from future enrollment.

TEXAS EDUCATION FREEDOM ACCOUNTS (TEFA) INTEGRATION

If my family is awarded Texas Education Freedom Accounts (TEFA) funds (estimated at approximately $10,474 standard per student for 2026-27, or higher for qualifying disabilities), these funds are placed in a state-managed Education Freedom Account. Parents authorize and direct payments from this account to Forms Academy Residency for tuition and other approved expenses.

• TEFA disbursements occur in installments: at least 25% by July 1, 2026; additional amounts by October 1, 2026 (cumulative at least 50%); and the balance by April 1, 2027.
• As funds are received and approved by the parent, they will be credited as payments against my student’s tuition balance.
• TEFA funds do not reduce Forms Academy Residency’s published tuition rate, alter the full-year binding commitment, or change the non-refundable nature of application/enrollment fees or pre-payments.
• I remain fully responsible for the entire annual tuition amount, including any shortfall if TEFA funds are delayed, prorated, insufficient, or not fully approved.
• I agree to coordinate with the FR Business Office after TEFA award notification (typically April–June 2026) to ensure smooth application of credits. Full tuition setup and payments (remaining balance after August pre-payment credit and
• TEFA applied) will occur after TEFA funding resolution, with the full balance due by December 31, 2026.

TERMS OF AGREEMENT

Financial Obligation

I agree to pay the full amount of all tuition, fees, and charges assessed by FR, including any late fees, finance charges, and collection costs incurred as a result of non-payment. I understand that failure to pay any amount when due may result in immediate cancellation of my child(ren)’s enrollment. Withdrawal from FR does not relieve me of my financial obligations, and I remain responsible for any outstanding payments due.

Default & Consequences of Non-Payment

If I fail to make a payment by the due date, FR may declare my entire outstanding balance, including principal, interest, late fees, and collection costs, immediately due and payable. If payment is not made within 15 days after receiving written notice of default, FR reserves the right to:

• Assess late charges and interest at a rate of 12% annually.
• Restrict access to records, including transcripts and enrollment privileges.
• Pursue legal collection efforts, including engagement of collection agencies and legal counsel. I authorize the FR Business Office to restrict access to my records and prevent future enrollment if I fail to comply with the payment obligations outlined in this agreement.

Payment Methods

Payments should be made through JotForm or any authorized payment terminal used by FR using a credit card, debit card, checking, or savings account. This is the preferred method of payment at FR. Credit card payments must be processed via JotForm or official invoices issued by FR. For security purposes, no payments should be given to unauthorized staff members.

For the August Tuition Pre-Payment ($1,375 for Primary/Lower Secondary or $2,050 for Upper Secondary, check rate), families may pay in full at the time of application or spread the cost over monthly installments up to June 2026. The monthly spread option is available only for online credit/debit card payments (with standard processing fees included in the credit card rate). Check payments must be made in full (no monthly spread option). The number of months available for spread depends on the application submission date—the earlier you apply and begin paying, the more months you can spread the cost (e.g., apply in February = up to 5 months; April = up to 3 months). This pre-payment is non-refundable upon acceptance and fully credited toward the total annual tuition. If not accepted, it is refunded in full.

Changes to Terms & Financial Policies

FR reserves the right to modify financial policies, including finance charges and payment terms, at its sole discretion. Any modifications affecting future payments will be communicated in writing to the address or email on file before taking effect.

I am responsible for informing FR’s Business Office of any changes to my name, address, telephone number, or email in a timely manner.

Credit Reporting & Collections

By signing this agreement, I grant FR permission to report my payment history to credit bureaus. I understand that failure to comply with payment obligations may result in negative credit reporting.

In the event of default, I agree to pay all collection costs incurred by FR, including but not limited to:

• Collection agency fees
• Legal filing and service fees
• Attorney fees and court costs
• Credit bureau processing fees
• Any other reasonable costs associated with debt collection

Acknowledgment & Agreement

By signing below, I certify that:

• I have read, understood, and agreed to the terms of this Financial Agreement.
• I acknowledge that my financial obligations remain binding, regardless of any changes to my child(ren)'s enrollment status.
• I accept that this agreement shall be enforced in accordance with the laws of the state of Texas.

FORMS ACADEMY RESIDENCY LOCATIONS & TUITION INFORMATION

2026–2027 Academic Calendar
Staff work days and teacher in-service: August 10–11, 2026.
Classes begin August 12, 2026 (full month of August instruction, including two full weeks before Labor Day break) and end May 27, 2027. This schedule aligns with a traditional 10-month billing year (August–May), ensuring steady program support and full August educator compensation.

Primary School (Grades 1–7) 

Held at our dedicated Training Grounds in Melissa, Texas. Requires active membership in Forms Academy.

• Annual Tuition: $13,750 (check) or $14,200 (credit card)
• Monthly Installments (if applicable): $1,375 (check) or $1,420 (credit card)
• Academic Resources & Materials Fee: $495 (annual)
  This single annual fee ensures every student starts the year fully equipped with the high-quality curriculum, books, digital tools, and essential materials needed for academic and athletic success. It replaces last year's $300 Supply Fee and covers increased costs from publishers and providers (e.g., Imagine Learning, IXL, NWEA MAP, Saxon Math, Generation Genius, novels, workbooks, classroom supplies, and program equipment) — all purchased in advance for the full year. Non-refundable if accepted; refunded if not accepted. Separate from tuition and not eligible for TEFA reimbursement.
• August Tuition Pre-Payment (due at application): $1,375
  This covers your student's first month of tuition (August 2026) as part of the full annual commitment. Texas Education Freedom Accounts (TEFA) Information

TEFA provides approximately $10,474 per student annually for private school students (higher amounts possible for qualifying disabilities with proper documentation). Funds are disbursed quarterly by the state (at least 25% by July 1, 2026; cumulative 50% by October 1, 2026; remainder by April 1, 2027) into a parent-directed Education Freedom Account. Parents approve payments directly to Forms Academy Residency for approved expenses, including tuition, required fees, textbooks/materials, technology (up to 10% cap), uniforms, meals, and transportation.

• TEFA funds may cover a significant portion of tuition and eligible fees/materials upon verification and approval.
• Families remain fully responsible for any remaining balance (the difference between total tuition and TEFA funds received/applied).
• Application Fee ($45), August Tuition Pre-Payment, and Academic Resources & Materials Fee ($495) are not covered by TEFA and are due at the time of application submission (non-refundable, except pre-payment refunded if not accepted).
• Full annual tuition balance (less August pre-payment credit and any TEFA funds applied by that date) is due in full by December 31, 2026 (end of first semester).
• TEFA-funded families: No monthly installment option; full remaining balance due by December 31, 2026, with TEFA quarterly credits applied as received.

Important Notes

• All tuition and fees are subject to change; any updates will be communicated in writing.
• The August Tuition Pre-Payment is non-refundable upon acceptance and fully credited toward August 2026 tuition as part of the total annual amount (does not reduce the published annual tuition rate).

• Total estimated first-year cost before TEFA credits: $14,290 (Primary) or $21,040 (Upper), including application fee and Academic Resources & Materials Fee (the August Tuition Pre-Payment is credited as part of the annual tuition).

How the Upfront Payment Works

When you submit your application, you pay three fees that help secure your child's spot and prepare for the school year starting August 12, 2026:

• Application Fee ($45) — Non-refundable.
  This covers the cost of processing your application, reviewing documents, and conducting the admissions evaluation (including soccer assessment if required). It is retained regardless of acceptance to cover administrative work already performed.
• Academic Resources & Materials Fee ($495) — Non-refundable if accepted; fully refunded if not accepted.
  This covers all required curriculum licenses (e.g., Imagine Learning, IXL, NWEA MAP), textbooks/novels, digital tools, classroom supplies, and program equipment — all purchased in advance on behalf of all students. Charging this fee now allows us to order everything before the school year begins, ensuring your child has every resource ready on day one.
• August Tuition Pre-Payment ($1,375 Primary / $2,050 Upper, credit card rate) — Non-refundable if accepted; fully refunded if not accepted.
  This is not an extra deposit — it is simply your child's first month of tuition (August 2026) paid in advance. It is fully credited toward your total annual tuition upon enrollment, reducing what you owe later.

 

Why this structure makes the most sense

By collecting these fees at application, we can:

• Cover immediate costs (evaluation, advance ordering of curriculum/books/equipment) without delaying your child's start.
• Fund full August payroll and program setup from day one.
• Avoid a large lump-sum payment at the beginning of the school year, when families are already managing uniforms, supplies, and back-to-school expenses.
• Reward early applicants with the option to spread the August pre-payment over several months (online card only), making the monthly impact much lighter.

This keeps your total first-year cost lower at enrollment and spreads the financial load more evenly — while ensuring the program is fully prepared and staffed when classes begin.
