  • Hear Us Students - Share Your Experience Anonymously

  • This form is completely anonymous and not connected to the university or any school system.

No identifying information (like your name, email, IP address, or student ID) is collected unless you choose to add it. You may skip any question or stop at any time.

    Why this form exists:

We want to center your voice, your experience, and your safety. Your story is important, and if you choose to share it, it may help empower others and support broader accountability among the university.

The purpose of sharing anonymous stories, when consent is given, is to raise awareness, validate survivor experiences, and encourage conversations about safety and accountability. This form does not make findings, determinations, or allegations, and sharing is never intended to replace formal reporting options.

  • Some individuals have expressed a desire for broader awareness of survivor experiences so that patterns can be recognized and conversations about safety can happen more openly.

If you are comfortable, you may choose whether or not we can share parts of your story anonymously on social media for awareness purposes only.
