By checking the box below and signing, I confirm that I am the authorized signer (the adult Participant or the parent or legal guardian of a minor Participant) and that this document binds me, the Participant (including any minor Participant on whose behalf I sign), and our respective heirs and legal representatives, and agree to the following:
- INTEREST & CONFIRMATION. This registration is an expression of interest only to participate in the Parade of Cultures, Heritage Fashion Showcase, and related activities (collectively, the “Event”). Participation is not confirmed until Bodhi Meditation Silicon Valley USA (the “Center”) confirms scheduling.
意向登记与确认。 本报名仅为参加“多元文化巡游、传统服饰文化秀”及相关活动（以下统称“本活动”）之意向登记。须待菩提禅修美国硅谷禅堂（Bodhi Meditation Silicon Valley USA，以下简称“禅堂”）确认排期并通知后，方视为参加确定。
- INDEMNITY / HOLD HARMLESS. To the fullest extent permitted by law, I agree to indemnify and hold harmless the Center and its directors, officers, employees, volunteers, contractors, and agents from any claims, damages, losses, and expenses arising out of or related to my and/or the Participant’s participation in the Event, except to the extent caused by the Center’s gross negligence or willful misconduct.
赔偿与免责。 在法律允许的最大范围内，如因本人及／或参加者参与本活动而引起任何索赔、损害、损失或费用，致使禅堂或其董事、主管、员工、志愿者、承包商或代理人遭受损失或承担责任，本人同意向其作出赔偿，并使其免受损害；但因禅堂重大过失或故意不当行为所致者除外。
- PARTICIPATION COMMITMENT. The Participant agrees to participate in the Event as arranged by the Center. The Center will determine and communicate the Participant’s Event schedule in advance. The Participant agrees to arrive at least forty-five (45) minutes before the scheduled participation time for check-in and preparation.
参加承诺。 参加者同意按照禅堂安排参与本活动。禅堂将统一安排并提前通知参加者的活动时间。参加者同意于预定参与时间前至少四十五（45）分钟到场签到及准备。
- ASSUMPTION OF RISK / LIABILITY WAIVER. I represent that I and/or the Participant are physically able to participate in the Event. I understand that participation in the Event involves inherent risks. I voluntarily assume all risks associated with my and/or the Participant’s participation in the Event, including, but not limited to, injury, illness, falls, contact with other participants, weather conditions, and loss of or damage to personal property. To the fullest extent permitted by law, I waive and release any claims against the Center and its directors, officers, employees, volunteers, contractors, and agents arising out of or related to my and/or the Participant’s participation in the Event, except to the extent caused by the Center’s gross negligence or willful misconduct.
风险自担／责任豁免。本人确认本人及／或参加者身体状况适合参加本活动。本人理解参与本活动存在固有风险。本人自愿承担本人及／或参加者参与本活动所产生的一切风险，包括但不限于受伤、疾病、跌倒、与其他参加者接触、天气因素，以及个人财物遗失或损坏等风险。在法律允许的最大范围内，本人放弃并解除对禅堂及其董事、主管、员工、志愿者、承包商及代理人因本人及／或参加者参与本活动而产生或与之相关的任何索赔、责任或请求；但因禅堂重大过失或故意不当行为造成者除外。
- MINOR SUPERVISION. A parent or legal guardian must accompany and supervise any minor Participant at all times during the Event. The Center does not provide childcare or supervision.
未成年人监护。 未成年参加者在活动期间须由父母或法定监护人全程陪同及照管。禅堂不提供托管或看护服务。
- EMERGENCY TREATMENT. In an emergency, I authorize the Center to obtain or arrange for emergency medical care for me and/or the Participant, at my expense.
紧急救治。 如遇紧急情况，本人授权禅堂为本人及／或参加者安排或协助取得紧急医疗救护，相关费用由本人承担。
- MEDIA RELEASE. I authorize the Center to photograph, film, and/or record me and/or the Participant during the Event and to use, reproduce, publish, and distribute such photos, videos, and/or audio recordings for Center-related purposes without notice or compensation.
媒体授权。 本人授权禅堂在活动期间对本人及／或参加者进行拍照、摄像和／或录音，并可为禅堂相关用途使用、复制、发布及传播上述照片、视频及／或录音，无需另行通知或支付任何报酬。
- LEGAL. California law applies; venue is Santa Clara County, California. If any part is unenforceable, the rest remains effective. This document is provided in English and Chinese; in the event of any inconsistency, the English version controls.
法律适用。以上条款与免责声明／豁免书适用加利福尼亚州法律；争议管辖地点为加利福尼亚州圣克拉拉县。若本条款任何部分被认定为不可执行，其余部分仍继续有效。本文件以英文及中文提供；如两种文本有任何不一致，以英文版本为准。
