2025-26 Women's Leadership Awards Program

The Women’s Leadership Awards Program is an optional program that is in addition to the LEAD Program forms. This form can be completed by a county WLC member or a county MSR and is due Tuesday, Sept. 15, 2026.

Instructions:

1. Only 1 form submission per county.

2. 15 questions total.

3. The points available for each question are listed.

4. To save form and return later:
- Click- Save at the bottom of the form.
- Click- Skip Create an Account.
- On the next screen you will see: Your form has been saved as a draft. Enter your email and a link will be emailed to you. This link will be needed to continue the form at a later time.
*We always recommend typing your long answers in a Word document and saving to your computer, and then copying and pasting into the form, to avoid losing any of your entry.
