My Empowering Impact, llc (MEI) Membership – Legal Disclaimer
The MEI Wellness Membership is an access-based discount program designed to provide members with access to reduced self-pay rates for eligible mental health and supportive services. This membership is not insurance, is not a prepaid therapy plan, and does not include clinical services.
Eligibility & Age Requirement
Membership is available to individuals 18 years of age or older.
For individuals under 18, membership enrollment and participation require parent or legal guardian consent and must comply with applicable state laws and clinical requirements. Client must be at least 5 years of age.
Each membership is individual and non-transferable. Membership benefits apply only to the enrolled individual and may not be shared, transferred, or used by any other person, including family members or dependents, unless otherwise approved in writing by MEI.
NO Substitute for Medical or Clinical Care
Participation in the MEI Wellness Membership does not substitute for medically necessary treatment, higher levels of care, or services determined to be clinically appropriate or medically necessary. All treatment recommendations, referrals, and levels of care are based on clinical judgment, medical necessity, and professional standards, regardless of membership status.
Membership discounts are intended for episodic, maintenance, and access-based care only and may not be appropriate for individuals requiring intensive, frequent, or higher-acuity treatment. When clinically indicated, referrals to higher levels of care will be made.
Availability & Scheduling
All services are subject to provider availability, scheduling capacity, and clinical appropriateness. Membership does not guarantee appointment availability, service frequency, specific providers, or immediate access to care.
Emergency & Crisis Services
The MEI Wellness Membership is not an emergency or crisis service. In the event of a medical or mental health emergency, members should call 911, go to the nearest emergency room, or contact local crisis services immediately.
Billing, Insurance & Payment
The monthly membership fee provides access to discounted self-pay rates only and does not cover or include clinical services. The membership fee is charged regardless of use or non-use, is non-refundable, and must remain active for discounted rates to apply.
Membership does not replace insurance, Medicaid, Medicare, or other payer coverage. Services covered by insurance or public programs will be billed separately in accordance with applicable laws and regulations. No services are double-billed.
If the membership fee is not successfully processed, canceled, or withdrawn, discounted rates will no longer apply and standard self-pay pricing will be charged for services rendered.
Program Modifications
MEI reserves the right to modify, suspend, or discontinue the membership program, pricing, eligibility requirements, or terms at any time, with reasonable notice, in accordance with applicable laws and regulations.
Acknowledgment
By enrolling in the MEI Wellness Membership, members acknowledge and agree that the membership program provides access to discounted pricing only, does not guarantee services, does not replace medically necessary treatment, and is subject to availability, clinical or supportive appropriateness, and professional standards of care. The memebership does not serve as treatment or proof of treatment. The membership does not subsitute any mental health care, crisiss or emergencies and if you are in need of help and/or have an emergency or mental health crisis you should dial 911 to help you.
