  • Thanks for taking an interest in making your community a healthier place!
The West Wimmera Health Service Community Health and Wellbeing Grants are designed to help community members with some funds, and with the support of the WWHS Health Promotion team, to do practical things that address the following regional priorities:

    Healthy Eating

    Living an Active Life

    Increasing Wellbeing (Staying Socially Connected)

Reducing Tobacco and Alcohol Related Harm

No matter how big or small your idea, please contact Jake Wiffen (WWHS Health Promotion Team Lead) before completing this form. Jake will be more than happy to discuss your idea and help you to develop the concept... and even to help fill in this form!

Contact Jake by phone 5391 4190 or email health.promotion@wwhs.net.au

  • Which of the following towns/areas will be covered by your project? (you may choose more than one)
  • As you fill in the boxes below, think about the four regional priorities:

    Healthy Eating

    Living an Active Life

    Increasing Wellbeing (Staying Socially Connected)

Reducing Tobacco and Alcohol Related Harm

Your idea may address one or more of these priorities directly or indirectly. The WWHS Health Promotion team can help you to develop your idea so that you can contribute to making the Wimmera and Southern Mallee healthier in the same way that our health services and local governments do.

We are all in this together!

  • Will your project engage with any of the following groups? (you may choose more than one, but you don't need to engage with all of them)
  • And now for the budget! It looks worse than it is...
Think about what you will need to spend money on to make your project work.

If you already have some funds from your group or organsiation, you can add that cash to the 'other funds' column.

Have a think about what you might use the WWHS grant funds for. That goes in the column for 'WWHS funding'.

If your group or organsiation can help this project to succeed by, for instance, pitching in time and energy, providing a venue to meet or materials for a project, then think about what that might be worth and add that to the 'in kind support' column.

Don't forget, Jake (health.promotion@wwhs.net.au) is here to help with filling in this form. No need to puzzle this out on your own!
