I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Lifeline Dog Rescue animal fostered by me.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Lifeline Dog Rescue and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I accept full responsibility for the dog(s) actions at all times, and release Lifeline Dog Rescue from any liabilities or damages that may be incurred because of fostering such dog(s).

I agree to have Lifeline Dog Rescue complete reference call checks to be able to approve my foster application.

I agree that if I'm unable to foster the dog anymore that I will will Lifeline Dog Rescue know right away. I understand that they are foster based and I will need to hold onto the dog until another placment can be found.

I agree that this is not my dog and Lifeline Dog Rescue is the owner of the dog. If Lifeline Dog Rescue asks for the dog back, I agree to comply immediately with thier request. If I would like to adopt the foster I will go through the application process and Lifeline Dog Rescue will determine if I am able to adopt the dog.
