    Participants must be above 16 years of age, but anyone under the age of 18 as of February 21, 2026, at 12:00 AM PST must have this form completed by a parent or legal guardian. This requirement must be fulfilled prior to participation in our event at OMSI. The deadline of this form's submission cannot be later than February 21, 2026, at 12:00 AM PST.   • Part II: To be completed by a parent or legal guardian of the participating volunteer -

  • All parents or legal guardian(s) must read the following term of participation carefully before signing on the bottom of this form. This form has to be completed and submitted by no later than February 21, 2026, at 12:00 AM PST.

    I, the undersigned parent/guardian, agree to the following for my child’s volunteering work at the 75th Oregon Agate and Mineral Annual Show at OMSI's Auditorium, between the February 23rd to the March 1st, 2026. 

    Self-Sufficiency: My child must bring and provide their own medication, food, and water. OAMS provides no refreshments unless the volunteering shift exceeds 4 hours.

    No Compensation: This is an unpaid position. Participation includes free admission to the show, as well as an entry into a prize draw but no monetary benefits or expense reimbursements.

    School Requirements: The volunteer is responsible for obtaining school permission and ensuring any extra credit is processed by their teacher.

    Transportation & Pickup: The parent(s) or the legal guardian(s) are solely responsible for the child’s transportation and timely pickup. OAMS provides no supervision or services once their work shift at the event ends.

    Release of Liability: I hereby release OAMS, its board, members, and affiliates (including OMSI and Rice NW Museum) from all liability for injury, illness, or loss of personal property.
