Liability Waiver & Assumption of Risk Agreement
I acknowledge that my participation as a counselor with the Florida Show Choir Intensive (FSCI) involves certain inherent risks, including but not limited to physical activity, travel between venues, participation in rehearsals and performances, and interaction with students and staff.

I voluntarily assume all risks associated with my participation and hereby release, waive, and hold harmless the Florida Show Choir Intensive, its directors, instructors, staff, volunteers, sponsors, and affiliated venues from any and all claims, liabilities, injuries, illnesses, losses, or damages that may occur during or as a result of my participation.

I understand that my participation is voluntary. I acknowledge that FSCI and its staff are not responsible for loss, theft, or damage of personal property. I authorize FSCI to seek medical treatment for me in the event of an emergency, and I understand that I am responsible for any medical expenses incurred.

Code of Conduct & Professional Expectations Agreement
I understand that the Florida Show Choir Intensive strives to provide a safe, positive, and professional environment for all participants. As a counselor, I acknowledge that I am a representative of FSCI at all times during the duration of the camp.

I agree to conduct myself in a professional, respectful, and responsible manner toward students, instructors, staff, hotel personnel, and the public. I will arrive on time, follow instructions, and contribute positively to the learning environment.

I understand that inappropriate language, bullying, harassment, disruptive behavior, or any form of disrespect will not be tolerated. These expectations apply during rehearsals, scheduled activities, breaks, off-site activities, and in shared or public spaces, including hotel and pool areas.

I agree to wear appropriate attire at all times while participating in or representing FSCI, including during breaks and off-duty hours when in shared or public spaces. I acknowledge that my behavior in person and online reflects upon the program.

I understand that failure to adhere to this Code of Conduct may result in disciplinary action, including dismissal from the program without compensation or reimbursement.

I grant permission to the Florida Show Choir Intensive (FSCI) to photograph, record, and/or film my image, likeness, and/or voice during activities associated with the program.
