Adopter has paid the sum of Amount donation. This donation will be used to prepare more animals for adoption, therefore not refundable.

The undersigned Adopter agrees to be bound by the following terms and conditions: Please initial by each paragraph.

Adopter will provide, all at his or her own expense, proper and sufficient food, water, shelter, kind treatment and proper medical care for the dog at all times. With this contract, you are agreeing to provide monthly care, yearly vaccines. During the lifespan of the dog.

Having a dog may require preparation such as putting up a fence to keep him safe and putting away things that your new dog may want to chew. Adopter will keep the dog as an in-home companion and not as an outdoor yard dog. All of our dogs need to come inside to sleep at night. You agree not to chain your dog keep in a yard or a crate longer than is appropriate for the individual dog’s age, breed, and need for activity. Adopter agrees to comply with all city, state, and federal laws relating to animals, including and not limited to "leash laws."

If the Adopter fails to comply with the terms of adoption at any time during the life of the dog, the dog must be returned to our agency.

The dog may not be given away or sold without the written approval of our agency, nor may it be surrendered to a shelter or pound. If it is decided that the dog is to be returned DSR needs seven days notice.

Adopter is fully aware that the agency makes no guarantees whatsoever as to the health, temperament, mental disposition and training of the dog. We can tell you what we know, with our knowledge and experience with the dog. Adopter understands that adopting a dog. This dog will need training, patience and time to adjust. Adopted is agreeing to help this dog fit into their family and give him/her time to adjust and acclimate. Returning the dog will cause harm to the dog’s ability to adjust again.

The dog must wear a collar and an information tag at all times. ID tag must be purchased upon adoption.

If the dog for any reason is at a shelter and is not claimed within 3 days by his/her owner, the owner will pay DSR $500 to go to the shelter and claim the dog. This fee will be paid within 7 days. The dog will not be returned to owner if DSR claims it.

I have read this dog adoption contract and fully understand it and agree to be legally bound by its terms and conditions.
