Bossy Paws Rescue
Spay/Neuter and Vaccination Responsibility Agreement
This Agreement is entered into between Bossy Paws Rescue (“Rescue”) and the undersigned Adopter in connection with the adoption of the animal described below.
1. Spay/Neuter Appointment Compliance
The Adopter understands and agrees that Bossy Paws Rescue will schedule a spay/neuter appointment through its designated veterinary provider.
If the Adopter misses, cancels, or fails to attend the scheduled spay/neuter appointment arranged by the Rescue for any reason, the Adopter agrees to:
Assume full financial responsibility for having the adopted pet spayed or neutered at their own expense.
Ensure the procedure is completed in a timely manner by a licensed veterinarian.
2. Use of Adopter’s Own Veterinarian
If the Adopter chooses to use their own veterinarian for spay/neuter services:
The procedure must be completed no later than the adopted pet reaching 7 months of age.
The Adopter must provide written proof from the veterinarian (invoice or medical record) to Bossy Paws Rescue confirming the procedure has been completed by that time.
Failure to provide proof by 7 months of age may be considered a breach of this Agreement.
3. Vaccination Responsibility
Once the adoption is finalized, the Adopter acknowledges and agrees that they are fully responsible for:
Scheduling all future veterinary appointments.
Covering the cost of all additional or ongoing vaccinations, boosters, and routine medical care required for the pet.
4. Communication and Scheduling
The Adopter agrees to remain in active communication with the Bossy Paws Rescue adoption coordinator and is responsible for:
Responding to messages regarding scheduling.
Providing updates as requested.
Coordinating and complying with all required appointments.
Failure to maintain communication may result in delays or forfeiture of Rescue-provided services.
5. Acknowledgment
By signing below, the Adopter confirms they have read, understand, and agree to comply with all terms outlined in this Agreement.
