  • Insurance:
    Our practice verifies your insurance eligibility and coverage but it is your responsibility to understand your plan and limitations. We can help provide that information or guide you on how to obtain it. We will attempt to answer any questions we can about your insurance and when possible, we will assist in resolving complications dental claims. Please understand that we cannot speak on their behalf. Your insurance contract is an agreement between you, your employer, and insurance carrier. In the event that your insurance company has not paid (on your behalf), you will be responsible for any unpaid balance on your or any covered dependents account.

    No insurance?
    You will be responsible for payment prior to and or at the start of your visit. If you are not able to pay in full, or if your treatment requires several visits, you can discuss your options with a member of our team. Additionally, our practice offers a membership plan that includes lower fees and complementary services. If interested, please inquire with a team member for more information.

    I, the insured, have read the above information and understand the policies regarding insurance I agree to comply and to be responsible for payment of all services provided. I understand the office will bill my insurance and follow up on any problems related to my claim and provide additional time for claim processing if needed.

  • Appointments at Mark Treystman, DDs, Inc

    Like most dental practices, we charge a fee for no shows, or last-minute canceling/rescheduling. When we make your appointment, we are reserving the doctor's and or hygienists’ time to serve your needs. We prepare and review your chart ahead of time, often in coordination with the labs and insurance companies.

    Additionally, other patients are awaiting on care and could possibly take your reserved time. We ask that if you must change an appointment to please give us at least 48 hours’ notice. There is a charge of $75 per hour scheduled. Our office manager may waive the fee on a case-by-case bases.
