Volunteer & Participant Release Waiver of Liability, and Assumption of Risk
PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT.
This Participant & Volunteer Release, Waiver of Liability, and Assumption of Risk (“Release”) is executed in favor of MISSION 514, Inc., a Georgia nonprofit corporation recognized as tax-exempt under Section 501(c)(3) of the Internal Revenue Code (“ORG”), and its directors, officers, employees, volunteers, agents, successors, and assigns.
1. Activities
I desire to participate in and/or volunteer with ORG in connection with programs, events, mission trips, retreats, service projects, meetings, travel, recreational activities, and related activities (collectively, the “Activities”).
2. Assumption of Risk
I understand that the Activities may involve physical exertion, travel, outdoor environments, manual labor, recreational activities, interaction with others, and other inherent risks, including risks of bodily injury, illness, permanent disability, death, and property damage.
I voluntarily and knowingly assume all such risks, whether foreseen or unforeseen, even if arising from the negligence of ORG.
3. Health Risks
I understand that participation may expose me to communicable diseases, including but not limited to COVID-19 or other viruses or bacteria. I knowingly assume this risk and release ORG from all liability related to such exposure.
4. Medical Authorization
- I authorize ORG to obtain emergency medical treatment on my behalf if deemed necessary.
- I understand that I am solely responsible for all medical costs, transportation, or evacuation expenses.
- I release ORG from any liability related to such medical treatment.
5. Release and Waiver of Liability
I hereby fully and forever release, waive, discharge, and covenant not to sue ORG from any and all claims, demands, causes of action, or liability of any kind arising out of or related to my participation in the Activities, including claims arising from the negligence of ORG, to the fullest extent permitted by Georgia law.
6. Insurance
I understand that ORG does not provide health, medical, accident, or workers’ compensation insurance for participants or volunteers. I am responsible for maintaining my own insurance coverage.
7. Media Release
I grant ORG the irrevocable right to photograph, record, and use my likeness, image, voice, or appearance in any media for lawful purposes, including promotion and fundraising, without compensation.
8. Conduct
I agree to conduct myself in a manner consistent with ORG’s mission and values and to comply with all rules, policies, and directions provided by ORG representatives.
9. Volunteer-Only Provisions (apply only if acting as a volunteer)
- I understand my service is voluntary and unpaid.
- I acknowledge ORG does not provide workers’ compensation coverage for volunteers.
- I agree to perform volunteer services responsibly and truthfully represent my skills and experience.
- I agree to maintain the confidentiality of sensitive or personal information encountered while volunteering.
10. Governing Law and Venue
This Release shall be governed by the laws of the State of Georgia. Any legal action shall be brought exclusively in the state or federal courts located in Fulton County, Georgia.
11. Acknowledgment
I understand that by signing this Release, I am giving up substantial legal rights, including the right to sue ORG.
