WAIVER AND RELEASE OF LIABILITY; INDEMNITY
Note: All CRC Watersheds Association Streams Cleanup participants must sign this form. Signature of parent or guardian is required if participant is less than eighteen (18) years of age. I understand that I (and/or my child) will be performing stream cleanup work as part of the CRC sponsored Streams Cleanup program. This work will be performed under the auspices and direction of a Site Coordinator, who is a volunteer working with the CRC organization. I have read, and I’m familiar with, the CRC Streams Cleanup General Program Conditions and Safety Guidelines provided to me by the Site Coordinator, and agree to comply with these conditions and guidelines. I understand that stream cleanup activities have a potential risk of injury. I agree that I (and/or my child) will only perform those activities that I am (and/or my child is) capable of doing and able to do, and I acknowledge and assume the risks of all such activities. I understand that participation in stream cleanup activities is strictly voluntary and that I (and/or my child) may stop the volunteer activity at any time. I hereby waive and release any and all claims and charges against CRC Watersheds Association or any of its agents, directors, officers, employees, volunteers, or representatives, for any damage or injury to me (and/or my child) or my (and/or my child’s) property, even if caused by negligence, as a result of my (and/or my child’s) volunteer participation in the CRC Steams Cleanup program. I further hereby release, indemnify and hold harmless the CRC Watersheds Association and all of its agents, directors, officers, employees, representatives, successors and assigns from any and all damages, injuries, claims and/or liability, even if caused by negligence, arising out of my (and/or my child’s) activities as a volunteer CRC Streams Cleanup participant. I give permission to the CRC Watersheds Association to include me (and/or my child) in any video productions or photographs that are made to document or publicize CRC’s activities.
