2026 Health + Housing Summit Request for Proposal Form

We’re seeking proposals to present for our 2026 Health + Housing Summit. Our theme this year is:

CARE | SYSTEMS | TOOLS

Now accepting proposals that advance health outcomes for people experiencing homelessness through stronger care delivery, smarter systems, and practical innovation.

SUBMISSION GUIDELINES:
- We welcome submissions from those working at the intersection of health and housing: healthcare providers, housing agencies, outreach teams, behavioral health organizations, public systems, community-based partners.
- Presentation timing: between 30min - 1hr.
- Application DEADLINE: Friday, May 8, 2026 | 5pm
