Swimming Pool and Spa Waiver Release Form

PLEASE READ CAREFULLY THE CONTENTS OF THIS FORM BEFORE SIGNING. RELEASE OF LIABILITY & SAFETY AGREEMENT In consideration of being allowed to use the swimming pool, spa, and related facilities at a Five Star Vacay property, I acknowledge and agree to the following:

NO LIFEGUARD ON DUTY: I understand that there is no lifeguard provided. Use of the pool and spa is strictly at my own risk and the risk of my guests.

SUPERVISION: Children under 18 must be supervised by a competent adult at all times.

SPA SAFETY: I acknowledge that high water temperatures in the spa can be dangerous for pregnant women, small children, and individuals with health conditions. I will limit soak time to 15 minutes.

GLASS & ALCOHOL: No glass containers are permitted in the pool area. Use of facilities while under the influence of alcohol or drugs is strictly prohibited.

CAPACITY & CONDUCT: I agree to abide by posted occupancy limits and will not engage in horseplay, diving, or running.

I accept full responsibility for the safety and conduct of the following guests in my party, for whom I am authorized to sign. I agree to hold Five Star Vacay and the property owners harmless from any and all claims or injuries arising from the use of these facilities.

LIABILITY RELEASE & INDEMNIFICATION

I agree to indemnify, release, and hold harmless Five Star Vacay, the Property Owners, and their respective affiliates from any and all costs, liabilities, claims, or damages arising from injury, illness, loss, or death—including those resulting from negligence—related to the use of the pool, spa, and facilities by myself or my party. I further release Five Star Vacay from any responsibility for loss or damage to personal property.

ACKNOWLEDGMENT of LEGAL STANDING

By signing below, I expressly declare that I am of legal age and authorized to sign on behalf of all guests in my party. I have read and fully understand the contents of this waiver and acknowledge that I am legally bound by its terms.
