I hereby authorize Full Stride Equine LLC dba Full Stride Equine and Livestock Therapy, dba Full Stride (“Full Stride”) to administer MagnaWave Pulsed Electromagnetic Field (“PEMF”) therapy to my animal listed on the form. I understand that PEMF therapy is intended to support circulation, reduce inflammation, relieve pain, and aid in overall performance and recovery, however it does not replace veterinary care.
I acknowledge that MagnaWave therapy is non-invasive and generally considered safe, however results may vary, and there is no guarantee of improvement or cure. I understand that PEMF creates more cell permeability and may increase absorption of medications, liniments, and topicals.
I confirm that my animal has received appropriate veterinary care and that I have disclosed any medical conditions, medications, injuries, and/or surgeries to the practitioner. I understand that PEMF therapy should be not be performed on animals with pacemakers, certain metal implants, or conditions where PEMF therapy is contraindicated.
By signing this form, I agree to fully and forever release and discharge, and defend, indemnify, and hold harmless Full Stride, its owners, practitioners, representatives, employees, and agents, and the MagnaWave company from all injuries (including death), losses, damages, claims (including negligence claims), demands, lawsuits, expenses, and any other liability of any kind, of or to me, my property or any other person, directly or indirectly arising out of or in connection with the PEMF services provided by Full Stride and/or their practitioners.
