Swinging for Scholars Registration Form
Swinging for Scholars, hosted by Israel Temple CME Church and MLC Charities , is a fundraiser event to provide scholarships for local high school students who have been accepted to post-secondary educational institutions. We are honored and consider it a privileged to be able to support these young minds as they grow on to all that God has for them and we thank you in advance for supporting them as well.

Please fill out the form below. There are four sections to choose from:
1. Golf Tournament Only (to just register your team),
2. Sponsor Package (to be an event sponsor),
3. Vendor Package (to sell a product or service or offer information at a booth) and
4. Donation Only (to just give to this event).
