2026-2027 Discovery School Student Application

I understand that Monday Enrichment is an optional, multi aged (3-4 & 5-7) add on and may be taught by an instructor other than my child’s Tuesday through Thursday classroom teacher. Monday Enrichment classes will also require a short list of supplemental materials selected by our Enrichment teachers. These materials may include items such as a Spanish workbook, sketchbook and sewing board. Families are responsible for purchasing these items in August, as enrichment materials are not included in the core academic program.

I confirm that my child is up to date on their immunizations and understand I will need to provide immunization records or an exemption form before the first day of school.

We understand and agree to a full and complete waiver and liability release on the part of Project Play S Corporation in connection with my child’s enrollment at the school. This includes my child’s participation in all activities, including but not limited to, the playground, and classroom activities. I understand and agree that this liability release will apply to my child’s entire attendance at Project Play Discovery School and participation in all the school’s activities. We authorize anyone working at the school to obtain medical care for my child and to transport my child to a hospital if in the workers opinion that medical care for my child is needed. We agree to pay all costs associated with the medical care including transportation, medical care, medication, and any other costs associated. We understand and agree that the school and its employees are not responsible for any costs incurred. We acknowledge that we have carefully read this form and understand and comply with all contents.

The below person(s) are able to pick up my child(ren) from school. This list can be updated through your Brightwheel App.

I give Project Play Museum consent to photograph or video my child and/or me or use photograph(s) or videos that already exist of my child and/or me that were taken in a childcare setting. I understand that the photographs, digital images, or video segments may be used in print or electronic media and that the photographs may be displayed on the school’s website, or social media pages. I give Project Play Museum permission to publish, exhibit, and distribute these materials. I understand that Project Play Professional Corporation owns the copyright to the multimedia material in which I, or my child may appear. Project play Discovery Museum will assure that it conveys positive images of children and reflect early childhood recommended practice. *For protection of privacy of the child, we guarantee that names will not be included.

I agree to promptly notify the school of any changes of the above information. I understand that I am responsible for the terms of this agreement. I understand and comply with all policies and procedures of Project Play Discovery School. Monthly payments due on the 1st of the month. We understand that circumstances may change, and families may need to withdraw their child from Discovery School. A minimum of two weeks' notice is required for withdrawal from Discovery School. This notice should be provided in writing to the school administration. No refunds will be issued for fees paid for the current period.

I agree and understand the policies and procedures listed in the Discovery School Parent handbook and will comply with the school’s rule and regulations. I understand that these policies and procedures listed in this handbook are subject to change to reflect the needs of the program. I understand I will be made aware of these changes in a timely fashion, and I will always adhere to the most up to handbook.

I understand that my Discovery School benefits include the following: complimentary admission to Project Play from August through May, early access to seasonal and summer camp registrations, and a ten percent discount on birthday party bookings.

There is no registration fee until your child is accepted into our program.
