DELIBERATE NUTRI SUPPLEMENT COMPANY
RELEASE OF LIABILITY, ASSUMPTION OF RISK & INDEMNIFICATION AGREEMENT
I understand and acknowledge that participation in athletic, fitness, wellness, bodybuilding, and related events promoted, sponsored, organized, or supported by Deliberate Nutri Supplement Company, its owners, officers, directors, employees, contractors, representatives, affiliates, partners, sponsors, and volunteers (collectively referred to as “Released Parties”) involves inherent risks, including but not limited to physical exertion, dehydration, muscular injury, cardiovascular stress, illness, permanent disability, property loss, and death.
I further understand that participation may include, but is not limited to:
Athletic competition
Training activities
Product sampling or supplementation
Demonstrations
Travel to and from the event
Exposure to environmental and facility-related conditions
I knowingly and voluntarily assume all risks, whether known or unknown, foreseeable or unforeseeable, arising from my participation in any event, activity, or promotion associated with Deliberate Nutri Supplement Company.
RELEASE & WAIVER
In consideration of being permitted to participate, I hereby waive, release, and discharge Deliberate Nutri Supplement Company and all Released Parties from any and all claims, demands, causes of action, damages, losses, or liabilities of any kind, including those arising from negligence, that may occur as a result of my participation.
This release applies to claims for:
Personal injury
Illness or disease
Property damage or theft
Economic loss
Emotional distress
Any other loss or harm
This release extends to activities occurring before, during, and after the event, including transportation to and from the event site.
ASSUMPTION OF RESPONSIBILITY
I certify that:
I am physically capable of participating
I have consulted a medical professional if necessary
I am responsible for monitoring my own health, hydration, and physical condition
I understand that supplements are not intended to diagnose, treat, cure, or prevent disease
I acknowledge that any supplements provided, promoted, or discussed are used at my own discretion and risk.
INDEMNIFICATION
I agree to indemnify, defend, and hold harmless Deliberate Nutri Supplement Company and all Released Parties from any claims, damages, costs, or expenses (including legal fees) arising from my participation or conduct, including claims brought by third parties.
MEDIA RELEASE
I grant Deliberate Nutri Supplement Company the irrevocable right to photograph, video record, or otherwise capture my likeness and to use such media for marketing, advertising, educational, and promotional purposes, without compensation or prior approval.
COVID-19 & COMMUNICABLE DISEASE ACKNOWLEDGEMENT
I acknowledge that participation in public events carries a risk of exposure to communicable diseases, including but not limited to COVID-19, and I voluntarily assume that risk and release the Released Parties from any related claims.
GOVERNING LAW
This agreement shall be governed by and interpreted under the laws of the State of New Jersey.
