Application Form

High School students interested in applying to attend the 2026 PHocus: A Summer Experience Camp for High School Students (Public Health: Outbreaks, Communities, and Urban Studies) summer experience must complete and submit this application and essay questions.
PROGRAM DATES AND LOCATION:
- July 27-July 31, 2026
- 9:00am - 4:00pm each day
- At Rutgers School of Public Health
  Piscatway, NJ 08854
APPLICATION DEADLINES:
- Early Decision Deadline: April 3, 2026
- Reguar Deadline: May 22, 2026
  (Applications received after the regular deadline will be reviewed on a space available basis.)
For early decision, applicants will be informed by April 30 and for regular decision, applicants will be informed by June 12. Upon acceptance, applicants must submit a non-refundable registration fee of $395 (scholarships available to those needing financial assistance) and enrollment forms.
