          I, the undersigned parent or legal guardian, acknowledge that participation in the MGPstudios Photography Camp involves activities that may include, but are not limited to:


          1. ASSUMPTION OF RISK
          I, the undersigned parent or legal guardian, acknowledge that participation in the MGPstudios Creative Photography Bootcamp involves activities that may include, but are not limited to:

          • Use of photography and lighting equipment
          • Indoor and outdoor photography sessions
          • Supervised photo walks
          • Handling of electronic and technical equipment
          • Standing, walking, and moving within studio or public spaces

          I understand that these activities carry inherent risks, including but not limited to minor injuries, equipment-related accidents, or environmental conditions. I voluntarily assume all such risks on behalf of my child.

          2. RELEASE OF LIABILITY
          I hereby release, waive, discharge, and hold harmless MGPstudios, its owners, directors, officers, employees, contractors, volunteers, instructors, and representatives from any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation in the program, including but not limited to personal injury, illness, property damage, or loss, except in cases of gross negligence or willful misconduct.


          3. MEDICAL AUTHORIZATION
          In the event of a medical emergency, I authorize MGPstudios staff to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

          I confirm that all medical conditions, allergies, and required medications have been disclosed on the registration form.


          4. EQUIPMENT RESPONSIBILITY
          I understand that my child may be permitted to use photography equipment owned by MGPstudios or their own personal equipment under supervision. I acknowledge that MGPstudios is not responsible for loss or damage to personal items brought to the program.

          Participants are expected to handle all equipment responsibly and follow all safety instructions provided by staff.


          5. BEHAVIOR & DISMISSAL POLICY
          I acknowledge that students are expected to act respectfully and follow all program rules and safety guidelines. MGPstudios reserves the right to dismiss any participant for unsafe, disruptive, or inappropriate behavior. Dismissal due to behavior does not qualify for a refund.

          6. REFUND & CANCELLATION POLICY
          I acknowledge that all registration fees are subject to the program’s stated refund and cancellation policy, which I have reviewed and understand.

          6a. CANCELLATION POLICY
          All registrations for the MGPstudios Photography Camp are considered final.

          Due to limited enrollment, advance staffing, and equipment preparation, no refunds will be issued for any cancellations, withdrawals, absences, or early dismissals for any reason, including but not limited to illness, scheduling conflicts, or personal circumstances.

          6b. TUITION CREDIT OPTION
          In place of a refund, tuition paid may be applied as a credit toward the following year’s MGPstudios Photography Camp, subject to the conditions below:

          -Credit is valid for one (1) calendar year from the original camp start date
          -Credit may be applied to one future camp session only
          -Credit has no cash value and may not be redeemed for cash or other services
          -Camp dates and availability for the following year are not guaranteed and are subject to capacity

          6c. MISSED SESSIONS
          Tuition credits will not be issued for:

          Individual missed days or partial attendance
          Early departures or late arrivals
          Dismissal due to behavior or code-of-conduct violations

          6d. PROGRAM CANCELLATION BY MGPSTUDIOS
          In the unlikely event that MGPstudios cancels a camp session due to unforeseen circumstances (including but not limited to low enrollment, weather, or circumstances beyond our control), families will be offered a $1,000.00 refund. If capmer bought a camera from MGPstudios, the camera will be theirs to keep.


          7. ACKNOWLEDGMENT & SIGNATURE
          I certify that I have read and fully understand this Liability Waiver & Assumption of Risk Agreement. I agree to its terms voluntarily and without reservation.
