Eye Appointment Details
Dr. Kice STRONGLY requests that you complete your OFA forms before your appointment.

Steps to Fill Out OFA Forms:

1. Click HERE to create an Owner Account or to login

2. Go to the Health Clinic Calendar tab

3. Select the orange event date from the Date/City column

4. Click the orange “*Register Button” for each animal

5. Select the "Online Veterinary Exam" check box at the bottom of each form

6. Click "Create" or “Create +/NEW” (to register additional dogs)

I, the undersigned, understand this reservation is accepted on a first-come, first-serve basis. I further understand there are NO Refunds (unless clinic is full) and I must arrive 20 minutes prior to my first scheduled appointment(s) to have time for check-in process. I understand failure to arrive 20 minutes prior to scheduled appointment that the SKC and CDADF may reschedule (if possible – but not guaranteed) and you forfeit your appointment all together.*
