April 2026 Player Registration Form
Program Info

Format: Indoor 3v3 mini tournament (small teams, 2 games at once for non-stop action!)

Live scoreboards, stat tracking, highlights, medals, & structured fun!

Group Size: 4 teams per age group (4 players per team; total 16 players per age group. Limited Spots!)

Focus: Competition · Skill application · Teamwork · Fun through games

Program Type: Co-ed – all players welcome regardless of gender or experience level

Each team will recieve 1st, 2nd, 3rd, & 4th place medals.

Schedule

3 weeks of tournament play:

Week 1: Friday April 10

Week 2: Friday April 17

Week 3: Saturday April 25

Daily Flow:

(U7–U9): 5:50–7:00 PM – Round-robin games (2 courts at

once) + fun challenges

(U10–U13): 7:00–8:30 PM – Round-robin + finals buildup (2 courts at once) + fun challenges

Week 1–2: Round-robin matches to build standings

Week 3: Playoffs + finals with group celebration!

Location: Rama Mask Gymnasium – 6147 Rama Rd, Orillia, ON L3V 0N7

Registration Fee

Fee: $60 per player; $10 sibling discount)

Payments: Handled directly by Coach Sam (E-transfer)

Policies

Commitment: Players should attend all sessions or notify the coach in advance if absent.

Equipment: Cleats or running shoes suitable for grass required. Shin guards recommended.

Uniforms: Pinnies provided for teams (wear comfortable athletic clothes).

Discipline: Zero tolerance for disrespect, rough play, or inappropriate language toward players, coaches, or volunteers.

Refunds: No refunds after program start.

Facility or Weather Issues: Any cancellations due to weather will be rescheduled when possible.

Safety: Any suspected concussion or injury must be reported immediately; a medical clearance may be required before return.

Photography & Media: Unless otherwise indicated on the registration form, player photos and videos may be used for promotional or social media purposes.

I release Simcoe Spartans, Simcoe Muskoka Soccer, it's organizers, coaches, volunteers, and facilities from any liability for injury, loss, or damage sustained during training activities. I accept full responsibility for any medical expenses that may arise from such injuries and confirm that my child is physically able to participate in outdoor soccer.
