1. Acknowledgment and Assumption of Risk
I, the undersigned, am the parent or legal guardian of the minor child listed below (“Participant”). I understand that participation in volleyball training sessions involves physical activity and the inherent risk of injury, including but not limited to sprains, fractures, concussions, and other potential injuries. I voluntarily assume full responsibility for all such risks, known and unknown, and whether caused by the actions or negligence of the Participant, other participants, coaches, or others.
2. Waiver and Release of Liability
I hereby waive, release, and discharge Carolina Rally, its owners, coaches, volunteers, agents, representatives, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness (including COVID-19 or other communicable diseases) that may be sustained by the Participant while participating in any Carolina Rally activity, whether occurring on or off the premises.
3. Medical Treatment Authorization
In the event of a medical emergency, I authorize Carolina Rally staff to obtain necessary medical treatment for the Participant. I understand that I am responsible for any and all medical expenses incurred.
4. Photo and Video Release
I grant Carolina Rally the right to take photographs and video recordings of the Participant during training sessions and related activities. I authorize the use of such media for promotional, marketing, and social media purposes, without compensation and without further consent. This includes but is not limited to publications, website content, and social media platforms.
5. Governing Law
This Agreement shall be governed by the laws of the State of South Carolina, without regard to its conflict of law principles. Any disputes shall be resolved in a court of competent jurisdiction within the state.
6. Acknowledgment of Understanding
I have read this Agreement in its entirety and fully understand its terms. I understand that I am giving up legal rights on behalf of myself and the Participant. I certify that I am the parent or legal guardian of the Participant and that I have the authority to sign this Agreement.
7. Carolina Rally Refund PolicyStandard Cancellations:
Participants who provide at least 3 days’ notice of absence will be eligible for a 50% refund of the session fee.
Emergencies:
If an emergency prevents attendance, please contact Carolina Rally directly to discuss receiving a credit toward a future session.
