THE SINGING LOTUS LLC/THE SINGING LOTUS SOUND BATH STUDIO 

We care deeply about creating a safe, supportive, and peaceful experience for everyone who enters The Singing Lotus Sound Bath Studio. The following waiver is here to ensure that you feel informed, supported, and clear as you choose whether or not to participate in any of our offerings. Please don't hesitate to let us know should you have any questions.

Liability Waiver & Release of Claims:

I affirm that I am at least 18 years of age and am signing this waiver on my own behalf. If I am signing on behalf of a minor under the age of 18, I affirm that I am the parent or legal guardian of that minor and have the legal authority to consent on their behalf and feel confident they are a good fit for the quiet environment of stillness. I understand that all participants, regardless of age, must be covered by a signed liability waiver prior to participation.

I acknowledge that I am voluntarily participating in a Sound Bath session and/or other activity offered by The Singing Lotus LLC. I understand that sound practitioners are not licensed healthcare or mental health professionals and do not provide medical, psychological, or diagnostic advice.

I understand that it is my responsibility to consult with my licensed healthcare providers (physical and mental health) to determine whether participation in a sound bath or other related service is appropriate for me/my condition. I acknowledge that this is especially important if I am pregnant or may be pregnant, have epilepsy or a seizure disorder, have metal implants, pacemakers, heart conditions, or any other medical or mental health concerns. 

I understand that sound baths and related sound or energy (Reiki) services are not a substitute for medical or mental health treatment, diagnosis, or care. I further understand that no specific outcomes, benefits, or results are guaranteed from participation in these services.

I acknowledge that while rare, sound, vibration and energy work may cause physical discomfort, emotional discomfort, or the resurfacing of memories or emotions that may require integration with a licensed therapist or healthcare professional. I understand that I am in full control of my participation and that it is my responsibility to inform the practitioner if I feel uncomfortable. I acknowledge that I may request that the session stop or that I may leave the session at any time.

I knowingly, voluntarily, and freely assume all risks, both known and unknown, associated with my participation, including but not limited to adverse effects, emotional responses, injury, illness, trauma, or disability that may arise during or after participation.

I agree to accept and assume all risks, actions, costs, damages, or liabilities arising from my participation, including those arising from the ordinary negligence of The Singing Lotus LLC, its owners, employees, volunteers, contractors, or guests.

I hereby release, waive, discharge, and hold harmless The Singing Lotus LLC, its owners, employees, volunteers, contractors, and guests (collectively referred to as the “Releasees”) from any and all claims, demands, causes of action, damages, or liabilities of any kind arising from my participation, including claims based on ordinary negligence.

I further agree that I will not bring any legal action against The Singing Lotus LLC, it's owner or the Releasees and that this release shall be binding upon me, my heirs, assigns, and legal representatives.

I understand that photographs, video, or audio recordings may be taken on rare occasions during sessions for educational or marketing purposes. I grant permission for my likeness to be used unless I notify The Singing Lotus LLC in writing prior to the session. I also understand that while The Singing Lotus LLC strives to maintain a respectful and private environment, other participants may occasionally take photographs or videos on personal devices without the knowledge or control of The Singing Lotus LLC. I acknowledge that the The Singing Lotus LLC is not responsible for images or recordings captured and posted on social media by other guests.

I understand and agree to the cancellation policy: any cancellation or rescheduling requires a minimum of 24 hours’ notice. Cancellations or rescheduling requests made with less than 24 hours’ notice may result in full payment being retained by The Singing Lotus LLC.

This agreement shall be governed by and construed in accordance with the laws of the State of South Carolina, and any legal action shall be brought in the appropriate courts of that state.

By checking the box below and attending a session or activity at The Singing Lotus LLC, I acknowledge that I have read and fully understand this Liability Waiver and Release of Claims. I understand that this agreement limits certain legal claims, and I agree to it's terms knowingly and voluntarily.
