The First Down Faith Foundation Football Camp will take place April 17th 2026 from 5:00 - 7:00 pm, with an optional Meet and Greet opportunity with Noah and Dash Fifita from 4:00 - 5:00 pm. This event is designed for middle school athletes, ages 11 to 14 years old, and provides a positive environment where young people can grow both on and off the field. Participants will experience a blend of football skill development, teamwork, and faith-based activities that encourage character, leadership, and confidence. Spots are limited to 100 - secure yours early!

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by The First Down Faith Foundation during the selected camp. In exchange for the acceptance of said child’s candidacy by First Down Faith Foundation ., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless First Down Faith Foundation . and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against First Down Faith Foundation . including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
Medical Release and Authorization

As the parent and/or legal guardian of the named minor, I acknowledge and agree that participation in First Down Faith Foundation activities involves inherent risks of physical injury. I voluntarily assume all such risks on behalf of my child.

I hereby release, waive, and hold harmless First Down Faith Foundation, its directors, officers, employees, volunteers, coaches, trainers, event partners, and affiliates from any and all claims, liabilities, damages, or causes of action arising out of or related to any injury, illness, accident, or loss that may occur during participation in any Foundation event or activity, whether caused by negligence or otherwise, to the fullest extent permitted by law.

In the event of an injury or medical emergency, I authorize event staff and volunteers to seek and obtain emergency medical assistance for my child, including transportation to a medical facility if deemed necessary. I understand that reasonable efforts will be made to contact me prior to any treatment, but that care may be provided if I cannot be reached in a timely manner.

I accept full financial responsibility for any medical expenses incurred on behalf of my child. I acknowledge that First Down Faith Foundation does not provide medical insurance coverage for participants.

This authorization and release is given freely and voluntarily and is effective for the duration of the First Down Faith Foundation event(s) on the date(s) registered.
Media Release and Authorization

I, the undersigned parent and/or legal guardian of the named minor participant, hereby grant First Down Faith Foundation, its representatives, employees, volunteers, partners, and affiliates permission to photograph, film, record, and otherwise capture the image, likeness, voice, and statements of my child during participation in Foundation events and activities.

I authorize First Down Faith Foundation to use such photographs, video, audio recordings, and digital content for purposes including, but not limited to: educational materials, promotional and marketing content, social media, website publication, newsletters, fundraising efforts, and community outreach.

I understand that:

No compensation will be provided for the use of such media;
The media may be edited, copied, published, or distributed;
Once published, the media may be accessible to the public;
I may request in writing that specific images be removed when reasonably possible.
I waive any right to inspect or approve the finished product and release First Down Faith Foundation from any claims, liabilities, or damages arising from the use of my child’s image, likeness, or voice as described above, to the fullest extent permitted by law.

This authorization is voluntary and remains in effect for the duration of the registered event(s) unless revoked in writing by the parent/guardian.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM PROVIDING MY ELECTRONIC SIGNATURE FOR THE MEDIA RELEASE AND AUTHORIZATION, INFORMED CONSENT AND ACKNOWLEDGEMENT, AND MEDICAL RELEASE AND AUTHORIZATION STATED ABOVE. I UNDERSTAND THAT THIS ELECTRONIC SIGNATURE HAS THE SAME LEGAL EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE AND IS EQUALLY AS BINDING.
