PREDRAWN TATTOO REQUEST FORM

Booking for March, April, May, June and July 2026

PLEASE READ:

This form is for requesting PREDRAWN designs only. For custom work please fill out the CUSTOM tattoo request form when it is open.
I require you to read through my Booking Policy and FAQ'S page prior to filling out this form.
Fill out this form completely and as detailed as possible, this is our consult.
You may submit multiple forms but please do a seperate form for each tattoo.
The person filling out the form must be the person wanting the tattoo.
Predrawn designs will not be tattooed more than once.
Predrawn designs are as is, no changes will be able to be made to these.
You can expect to recieve and email from me within two weeks of submitting.

Scheduling Process:
My pricing is $200/hour, minimum of 1 hour.
If approved, you will be contacted for scheduling and info within 2 weeks of submitting your form.
I require a deposit of $80 down to schedule and hold your appointment date. This deposit will be deducted off the total cost of the tattoo.
If you do not reply within 48 hours of being contacted with the information, your submission may be forfeited.
Deposits are NON-REFUNDABLE. If you do not show up to your appointment or you cancel within one week of your appointment time, your deposit will be forfeited and you will have to put down a deposit again to reschedule and to keep the rest of your appointments.
