My son/daughter has my permission to participate in all activities in the Berlin Football and/or Wrestling Youth Camp. My son is physically fit to participate in football and wrestling activities. I authorize Berlin Youth Camps to perform any emergency action deemed necessary in the case of an accident. By signing this registration, the undersigned on behalf of himself/herself and on behalf of any other legal guardian of the participant, agrees to hold the City of Delaware, the Olentangy Berlin School District, the Berlin Football and Wrestling Youth Camp and any of their employees harmless from any and all injuries or claims that may arise as a result of participation in this camp.
