Event Participation Waiver*
By submitting this form, I acknowledge and agree that:
- I give permission for DPIC to use my submitted information (name, email, phone, and RSVP details) solely for the event
- I am voluntarily participating in events organized by the Dunham Pointe Indian Community (DPIC).
- I understand that participation may involve physical activities, cultural programs, or other event-related activities, and I assume any risks associated with attending.
- DPIC, its organizers, volunteers, or affiliates will not be held liable for any injury, loss, or damage to myself, my children, or my property during events.
- I give permission for DPIC to use photos or videos taken during the event for community newsletters, social media.
- I agree to follow all safety instructions and community guidelines during the event.
