ICON Men's Ministry Drone Training Signup Form

This project consists of a structured drone training session designed to provide participants with foundational and operational knowledge of unmanned aerial systems (UAS). The training will focus on safe operation, regulatory compliance, and practical flight skills. Instruction will include both classroom-based learning and hands-on flight practice.
IMPORTANT: All participants must attend the mandatory safety briefing before the start of training. Participants are required to follow all instructor directions and comply with FAA regulations, organizational policies, and local operating restrictions at all times. Only approved drones and equipment may be used during the training.

- 1. Assumption of Risk
  I understand that operating Unmanned Aircraft Systems (UAVs or "drones") and participating in associated activities—including flight maneuvers, racing, and spectating (hereafter “Drone Activities”)—involve inherent risks. I voluntarily assume all risks of injury, death, or property loss that may result from Drone Activities organized or promoted by ICON Church.
  
  2. Release of Liability
  I AGREE TO RELEASE ICON Church, and its owners, affiliates, insurers, employees, and agents (collectively “Providers”) from all liability for injury, death, and property loss or damage that results from participation in Drone Activities, including liability resulting from the negligence or gross negligence of such Providers.
  
  3. Safe Operation & Compliance
  Legal Compliance: I will operate all drones in full compliance with FAA regulations and local laws.
  Privacy: I will not operate the drone in a manner that invades the privacy of others.
  Responsibility: I am solely responsible for the safe operation of the drone in my possession.
  
  4. Provided Equipment (Loaned Drones)
  If I am using a drone provided by ICON Church, I acknowledge the following:
  
  "As-Is" Condition: I accept the equipment in its current condition and have had the opportunity to inspect it for visible defects before flight.
  Competency: I represent that I have the necessary skill and physical ability to operate the provided equipment safely.
  Assumption of Mechanical Risk: I understand that electronic equipment can malfunction unexpectedly, and I assume the risk of such malfunctions during my use.
  
  5. Indemnification & Property Damage
  I agree to defend and indemnify Providers for all loss and damage arising from claims related to my participation. Furthermore, I agree to be financially responsible for any damage caused to drones or equipment owned by ICON Church if such damage results from my reckless operation or failure to follow instructions.
  
  6. Governing Law & Venue
  I acknowledge that the laws of the State of South Carolina govern this agreement. Any action must be prosecuted in the Superior Court for Richland County, South Carolina.
- I have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it be a complete and unconditional release of all liability to the greatest extent allowed by law.
