I / We, the undersigned parent(s)/legal guardian(s), acknowledge, understand and accept that there are inherent risks associated with participating in the activities of the Beltway Church of Christ Spring Camp Program, and that doing so could result in injury. We the parents(s)/legal guardians(s) release from liability (will not hold Beltway Church of Christ and the staff at the 2026 Spring Camp responsible) for any accident of bodily injury sustained, loss or damage of personal articles while on the premises or from the result of participating in any activity sponsored by Beltway Church of Christ Spring Camp. If an accident should occur, we give the camp and staff our approval/permission to take our child to a local hospital to receive medical attention. THE PARENTS OR GUARDIANS ARE RESPONSIBLE FOR PAYMENT AND HEALTH/ACCIDENT INSURANCE.

I / We grant permission to the Beltway Church of Christ Summer Camp 2026 for the use of photographic or electronic media images in any presentation. I understand that I may revoke this authorization in writing at anytime by notifying the Beltway Church of Christ Summer Camp(6000 Davis Blvd, Camp Springs MD 20746) in writing. This revocation will not affect any actions taken before the receipt of the written revocation. Images may appear on the Beltway Church of Christ Website will be stored in a secure location and only authorized staff shall have access
to them.
