  • Thank you for your interest in being a SHINE Team Captain this summer! We want your energy, ideas, and leadership. This is a chance to make a difference, grow as a leader, and help make SHINE an awesome week.

    Who can be a Team Captain: Students heading into high school through recently graduated high school seniors

    SHINE Kickoff & Training for Team Captains & Adult Volunteers: Date TBD
    SHINE Dates: July 13–17, 2026

    As a SHINE Team Captain, you will:

    • Attend the SHINE Kickoff & Training before SHINE begins so you know what to expect, meet the team, and feel ready to lead
    • Attend every day of SHINE and lead a team of 6th–8th graders
    • Help plan and lead games and fun activities
    • Be a role model during the week, especially during service work, Mass, and activities
    • Help keep track of your team and make sure everyone feels included
    • Share feedback each day to help us learn and make the week better
    • Be paired with at least one adult leader who will support you and assist you

    After we review all applications, we’ll reach out to let you know if we think you’d be a great fit for the SHINE Team Captain role this year.

  • Team Captain Agreement
    I, *   *, agree to enter into this agreement with Parent/Guardian and the St. Joseph Youth Ministry Leaders. This agreement is set forth to identify minimum expectations of a Team Captain for required activities and behaviors during SHINE Service Week, which will take place July 12-17, 2026.

    As a Team Captain in SHINE Service Week, I understand and agree to the following expectations:

    • Arrive at each event with a positive attitude, a willingness to participate, and an eagerness to have fun
    • Follow the guidance of all adult leaders, show respect in all situations, and represent myself, my parent/guardian, and the St. Joseph community with kindness and integrity.
    • Adhere to all safety procedures and guidelines during the event.
    • Refrain from using cell phones during SHINE, unless given permission from an adult volunteer.

  • Parent/Guardian Agreement
    As the parent/guardian of the St. Joseph Teen, I agree to the following:

    • I understand that the parish is not responsible for having medical personnel available during SHINE events or associated activities. However, parish staff may authorize or perform emergency medical treatment if they believe it is necessary for my child’s health, safety, and well-being. In the event that immediate medical examination and/or treatment is required, I hereby authorize the parish personnel to seek the necessary medical services for my child. I accept financial responsibility for any diagnosis, treatment, or medication deemed necessary.
    • I agree to cover all costs associated with any medical and dental services provided to my child under this authorization. This release form applies to the time period from July 12-17, 2026.
    • I authorize all SHINE adult volunteers to transport my child to service events.
    • I understand that neither the Catholic Bishop of Chicago, A Corporation Sole, nor St. Joseph Catholic Church in Libertyville will be held liable for any loss or damage to my child’s property prior to, during, or after the event, due to theft, fire, accident, or any other cause beyond their control.
    • I give permission for St. Joseph SHINE adult volunteers to contact my child via phone, text, email, and/or social media.
    • I authorize the Parish to use and reuse my child’s image, likeness, and voice in any photographs, video recordings, and/or audio recordings (collectively referred to as “Recordings”) for educational, marketing, or promotional purposes without compensation or prior notice. These materials may be used in the parish bulletin, on websites, social media, brochures, magazines, books, or other communications at any time.
