By signing Below, I agree and or understand as applicable that:

• That this is a loan of the items identified above from Border Animal Rescue (BAR)
• The identified items remain the property of BAR.
• I may not further loan or pass the items received to third parties.
• I will take reasonable precautions to prevent theft such as: keeping the above items out public view and storing inside of a building, in a concealed location, or storage shed when not in use.
• I will keep and return the items in clean and in good operational condition.
• The loan of the items is for 3 weeks from the date signed / received.
• If not actively trapping I will notify the BAR representative below for pickup.
• Extension of loan may be requested by contacting the BAR representative below.
• I will return the trap immediately if BAR requests that I do so.
• I have been trained and understand how to operate and care for items received.
• I assume full responsibility and liability for any injury both by the undersigned and any other individual within or outside of my household that may occur regarding injury form both or either the trap or animals trapped.
• I agree to hold harmless from any liability associated with the use by the undersigned and any other individual who gains access to the trap, both intentionally or unintentionally.

* Use of Information:
The information collected on this two sided form will be used by BAR to track the location of BAR property (as identified on this form) used in the performance of its mission. The exception being if "addition to mailing/email list" box is checked. Information will not be released to third parties, unless in the unlikely event it is required for the purpose of the reclaiming the items issued on this form.
