CCSA ROLLER SKATING WAIVER & RELEASE OF LIABILITYIn consideration of being permitted to enter, attend, observe, or participate in any activity at Central Coast Sports Arena (“CCSA”), including but not limited to roller skating, public skating sessions, birthday parties, private events, group rentals, and all related activities (collectively, the “Activities”), the undersigned participant and/or parent or legal guardian agrees to the following:ASSUMPTION OF RISKI understand that roller skating and activities within a skating facility involve inherent and unavoidable risks, including but not limited to falls, collisions with other skaters, contact with walls, railings, rink barriers, uneven or slick surfaces, fast-moving skaters, equipment failure, rental skate issues, and the unpredictable behavior of others. I voluntarily choose to participate and assume full responsibility for all risks, whether known or unknown, foreseeable or unforeseeable.RELEASE OF LIABILITYI hereby release, waive, and forever discharge Central Coast Sports Arena, its owners, officers, employees, volunteers, agents, contractors, event hosts, and representatives (“Releasees”) from any and all claims, demands, damages, actions, or causes of action arising out of or related to participation in the Activities, including injury, illness, death, or property damage, whether caused by negligence or otherwise, to the fullest extent permitted by California law.INDEMNIFICATIONI agree to defend, indemnify, and hold harmless the Releasees from any claims, liabilities, damages, costs, or expenses (including attorney fees) arising from my participation or the participation of any minor or guest listed on this waiver.SKATE RENTAL & EQUIPMENT RESPONSIBILITYIf renting skates or using personal equipment, I understand that proper fit, use, and condition of skates and equipment are my responsibility. I accept all risks associated with rental or personal equipment, including injury caused by improper fit, misuse, or equipment failure.RULES & SAFETY COMPLIANCEI agree to follow all posted rules, verbal instructions, and safety guidelines provided by CCSA staff. Unsafe behavior or rule violations may result in immediate removal from the facility without refund.MEDICAL AUTHORIZATIONI authorize CCSA staff to administer basic first aid and to seek emergency medical treatment if deemed necessary. I accept full financial responsibility for any medical care provided.COMMUNICABLE DISEASE ACKNOWLEDGEMENTI acknowledge that participation in public activities may expose me to communicable illnesses, including but not limited to COVID-19 and influenza, and I voluntarily assume all risks related to such exposure.PERSONAL PROPERTYCCSA is not responsible for lost, stolen, or damaged personal property, including phones, clothing, skates, or equipment.MEDIA RELEASEI grant permission for CCSA to photograph or record participants on the premises and to use such images or recordings for promotional or marketing purposes without compensation.BIRTHDAY PARTIES & PRIVATE EVENTSFor private events and parties, I acknowledge responsibility for the conduct and supervision of all guests listed on this waiver and accept responsibility for any damage to CCSA property caused by myself or my guests.MULTIPLE PARTICIPANTS / MINORSThis waiver applies to all skaters listed below, including minors. By signing, I certify that I am the parent or legal guardian of any minor listed and that I have the authority to execute this waiver on their behalf.ARBITRATION & VENUEAny dispute arising from participation in the Activities shall be resolved through binding arbitration in Santa Barbara County, California. I waive the right to a jury trial.ACKNOWLEDGEMENTI have read this waiver carefully, understand that I am giving up substantial legal rights, and agree voluntarily and knowingly.
