I, First Name* Last Name* , acknowledge receipt of one (1) S.T.A.R. Inc. orange safety jacket issued to me by S.T.A.R. Inc.I agree to return the issued jacket in good, usable condition immediately upon separation from employment with S.T.A.R. Inc., reasonable wear and tear excepted.If I fail to return the issued jacket upon separation from employment, I authorize S.T.A.R. Inc., to the extent permitted by law, to deduct the replacement cost of the jacket ($60) from my final paycheck.I understand that misuse of, damage to, or violations related to the issued jacket may result in disciplinary action, up to and including termination of employment.
