Spring Trash Bash Team Registration

Thank you for your interest in our semi-annual Trash Bash event! We only need ONE registration for your group but EVERY GROUP PARTICIPANT must fill out the SPECIAL EVENTS VOLUNTEER APPLICATION. Please register by 5:00 P.M. on April, 22, 2026. Check-in time is between 8:00 AM - 9:00 AM on Saturday, April 25th at the Town East Mall parking lot marquee. A confirmation will be sent to you by email on Friday with details for supply pickup. Day of registrations will be accepted onsite. Please print the above Volunteer form and have it completed for each person in your group. Feel free to call or email Jonathan Chitty, at 972-329-8331 or kmb@cityofmesquite.com with any questions. Contest items must be returned back to the mall by 11:00 AM. ALL TRASH AND TRASH BASH SUPPLIES MUST BE RETURNED BY 12:00 PM.PLEASE NOTE: EACH PARTICIPANT IS RESPONSIBLE FOR TRANSPORTATION TO/FROM THE CLEAN-UP LOCATION. NO TRANSPORTATION IS PROVIDED. Completion of the clean-up is around 11:00 a.m. and supplies are returned to Town East Mall by Noon. VOLUNTEER ACKNOWLEDGEMENT OF THE ALLIANCE DIRECT CONTRACTING PROGRAM: If I am hurt on the job (as a volunteer), I understand that: 1. I must choose a treating doctor from the Political Subdivision Workers’ Compensation Alliance (The Alliance) list of doctors designated as treating doctors. The most up-to-date list can be found online at www.pswca.org. 2. I must go to my treating doctor, chosen from the Alliance list, for all health care for my injury. If I need a specialist, my treating doctor will refer me. If I need emergency care, I may seek emergency treatment. 3. I may have to pay the bill if I receive health care from a doctor, other than an Alliance doctor, without approval from the Texas Municipal League Risk Pool adjuster. I have received information that tells me how to get on the job injury care under the City of Mesquite workers’ compensation coverage.*
