  • In accordance with ACAHM’s Teach-Out Policy, if a school voluntarily enters into a Teach-Out Agreement or if the Commission requires a school to submit a proposed Teach-Out Agreement as part of a Teach-Out Plan, the school must submit all information listed below in an electronic format, prepared in accordance with ACAHM’s Document Submission Instructions. 

    This form is to be completed by the closing program/institution ("Closing Institution") and must be submitted once for each institution they are entering into a Teach-Out Agreement with ("Teach-Out Institution").

  • 1. Student Enrollment Details

  • Provide detailed information, by program, regarding the currently enrolled students with the following columns:

    • Program Name
    • Student Name
    • Total Program Clock/Credit Hours
    • Program Requirements – Clock/Credit Hours Earned
    • Number of Outstanding Courses/Credits/Clock
    • Estimated Graduation Date

    This information should be provided in a spreadsheet or CSV file.

  • 2. Delivery Mode Change

  • If the Teach-Out Agreement represents a change in delivery mode (e.g., from residential to distance education), students must be provided an option via the same method of delivery as the original program. 

  • 3. Teach-Out Agreement

  • Attach a complete copy of the proposed Teach-Out Agreement between the closing institution and the teach-out institution. There must be a separate Teach-Out Agreement with each school that will participate in the teach-out of students.

  • The closing institution is required to provide copies of all notifications from the institution related to the institution's closure or to teach-out options to ensure the information accurately represents students' ability to transfer credits and may require corrections. Attach all notifications below.
