ADDITIONAL NOTE ABOUT CLASS PLACEMENT: Class age groups are combined for Early Care & After Care since enrollment is lower for those time frames. In After Care, children are required to have a rest time and our youngest children will nap. A 2nd snack is needed for children who will not nap during that time.
We agree that our child may attend Riverbend Christian School for early care (8:00-9:00) and/or after school care (1:30-3:30) as selected above. When the children is brought to school, he/she will be left in the presence of a staff member and will be released only to the parents or to those who the parents have provided written authorization to pick up the child. I acknowledge with my signature below.*

I understand that the days I choose for Early and/or After Care will apply to all sessions that I have enrolled my child. I understand that all Early & After Care fees will be due by June 5th.

In the event of withdrawal for any or all camp sessions, please refer to the camp registration form for withdrawal deadlines and processing fees. Any changes to early and/or after care enrollment must be made by June 5th. After June 5th, no refund will be given.

I understand that late fees will apply for late pick up should I fail to pick up my child by 3:30 p.m., if I'm signed up for After Care.
