#HalfMyDAF Commitment & Grant Nomination Form
Thank you for putting your charitable dollars to work by pledging to #HalfMyDAF!

This form has two parts: a commitment to #HalfMyDAF, and a section to nominate your grants for matching funds. After making your commitment, you can complete the nominations section as often as you'd like through June 18th to qualify for the first of two matching rounds this year, and through October 8th for the second round.

Please note that due to administrative costs, we're only able to match grants of $250 or more. (Feel free to list all grants, including smaller nominations, for our awareness.) If you have trouble submitting this form or uploading your grant confirmations, please email us at contact@halfmydaf.com.

Each nomination requires a few quick details and an uploaded confirmation of your grant. This upload can be a screenshot from your DAF platform, a copy of an email confirmation you receive, or a PDF you download from your DAF provider. Each additional grant confirmation you submit increases your chances of winning a match, let's go!
