"I understand that participation in DL6 Sports tryouts and related activities involves inherent risks, including physical injury. I voluntarily assume all risks associated with participation.
In consideration of being allowed to participate, I hereby release and hold harmless DL6 Sports, its owners, directors, employees, coaches, volunteers, and affiliates from any and all claims or liability arising from participation, including those caused by negligence, to the fullest extent permitted by law.

I certify that the participant is physically able to participate. In the event of an injury or emergency, I authorize DL6 Sports staff to obtain emergency medical treatment if necessary, and I accept responsibility for any related medical expenses.

I understand that DL6 Sports does not provide medical insurance coverage for participants.

I grant permission for DL6 Sports to use photos or videos taken during tryouts for promotional purposes.

This waiver is governed by the laws of the state in which DL6 Sports operates."
