Membership Application

Membership is required to access all services under the umbrella of Malena Energetics - A Temple of Light, formerly Malena Energetics Wellness Center. Being a member allows you to decide what type of services you want to receive, including modalities often not covered by traditional medical facilities. Only members with a scheduled appointment are permitted in the facility and treatment areas. Exceptions may apply to disabled persons and minors. As a private member, individuals agree to services, provider to member, which is out of the jurisdiction of the city, state and/or federal agencies. If there is something you don’t agree with, you don’t do it. If there is a claim or issue with a provider, a member agrees to private arbitration instead of going to the public medical boards, law enforcement, etc. All products and services are provided via an exchange process.

As of May 1, 2024, all card exchanges have a 2.9% fee. You can avoid fees by using Zelle, Venmo, Cash, or Check. RETURN POLICY: No Refunds

APPOINTMENTS: If you need to reschedule, please do so within 48 hours so we can offer your time to someone else. Exchange amounts subject to change. We reserve the right to refuse service.

As a member you understand the provider does not diagnose, treat, or cure any illness. Please discuss your personal health, including any options with your personal, qualified health practitioner before making changes to your diet or adjusting/discontinuing any medication. It is the responsibility of the member to do their due diligence. All member information is private and kept confidential.

As a member you agree to keep other members' information confidential when group sessions are accessed. Members can withdraw their membership at any time by contacting malenaenergetics@gmail.com.

I understand that products and services offered by Malena Energetics - A Temple of Light support the body, mind, and soul's natural ability to create wellness in my life. 

I understand that Malena Energetics - A Temple of Light facilitators do not diagnose conditions, nor do they prescribe or perform medical treatment, prescribe substances, nor interfere with the treatment of a licensed medical professional. 

I understand that energy healing, or any service provided by Malena Energetics - A Temple of Light does not take the place of medical care.  It is recommended that I see a licensed physician or licensed health care professional for any physiological or psychological ailment I may have. I understand that energy exchange provided by Malena Energetics - A Temple of Light can complement any medical or psychological care I may be receiving. 

I also understand that the body has the ability to heal itself and to do so, complete relaxation is often beneficial.  I acknowledge that long term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal itself.  Malena Energetics - A Temple of Light is not liable for my medical, mental, or emotional conditions before, during, or following any service provided by Malena Energetics - A Temple of Light.

I take personal responsibility for my well-being and with respect for myself I gratefully accept control of my choices.  My heirs, guardians, legal representatives, and I hereby and forever release, waive, and discharge any claims against, Malena Energetics - A Temple of Light, and/or any of their associates or affiliates. I take full responsibility for any loss or injury incurred while in association with Malena Energetics - A Temple of Light and/or any of their associates or affiliates.

I have carefully read this agreement and fully understand its content.  I am aware that this is a waiver and release of potential liability and a contract between the above noted parties and myself.  I understand that this contract is binding and acknowledge that I am signing this of my own free will.
