Participation Waiver
1. Assumption of Risk
I understand that participation in basketball tryouts, training, and related activities involves inherent risks, including but not limited to physical injury, illness, permanent disability, or death. I voluntarily choose to participate and assume all risks, known and unknown, associated with these activities.
2. Release of Liability
In consideration of being allowed to participate, I, on behalf of myself and/or my minor child, release, waive, discharge, and hold harmless Prospect Performance Basketball, Prospect Performance, LLC, its owners, directors, coaches, staff, volunteers, affiliates, landlords, and agents from any and all claims, demands, injuries, damages, or causes of action, whether arising from negligence or otherwise, to the fullest extent permitted by Florida law.
3. Medical Responsibility
I certify that the participant is physically capable of participating in basketball activities. I understand that Prospect Performance Basketball does not provide medical insurance, and I am solely responsible for any medical expenses resulting from injury or illness.
4. Media Release (Photos & Video)
I grant Prospect Performance Basketball the irrevocable right to photograph, video record, and otherwise capture the participant’s likeness, and to use such media for promotional, marketing, social media, website, and educational purposes without compensation or prior approval.
5. Conduct & Removal
I acknowledge that Prospect Performance Basketball reserves the right to remove any participant for unsafe behavior, misconduct, or failure to follow rules, without refund or obligation.
