Disclosure, Participation & Accountability Consent Statement

I, the undersigned applicant, hereby affirm that all information I have provided to Raising Wolvez™ and the Men’s Group is true, complete, and accurate to the best of my knowledge. I understand that providing false or misleading information may affect my eligibility or continued participation in the group.

I voluntarily request participation in the Raising Wolvez™ Men’s Group and acknowledge that this group is designed to support personal growth, accountability, leadership development, discipline, and character building. I understand that meaningful results require intentional effort, commitment, and consistent participation.

By signing this consent, I agree to:
• Actively and intentionally participate in all scheduled group sessions, activities, discussions, and program components to the fullest extent of my ability.
• Complete all assignments, exercises, reflections, and tasks as requested by group mentors, facilitators, or leadership.
• Demonstrate respect, accountability, and responsibility toward mentors, fellow group members, and program staff at all times.
• Accept constructive feedback and guidance as part of my personal development and growth process.
• Be held accountable for my attendance, participation, conduct, and completion of group requirements.

I understand that my participation in the group is a privilege and that failure to meet participation expectations, repeated absences, refusal to engage, or conduct inconsistent with group values may result in corrective action or removal from the group at the discretion of Raising Wolvez™ leadership.

I acknowledge that the Raising Wolvez™ Men’s Group is not a substitute for professional medical, psychological, or legal services, and that the group provides mentorship, guidance, and support—not educational instruction.
