Authorization and Credential Use Notice
By submitting this form, you expressly authorize HillTown Tech, LLC and/or K5 Solutions, LLC to access, use, and store the credentials provided for the sole purpose of performing requested services on your behalf.
You confirm that you are the authorized owner or administrator of the accounts associated with these credentials, or that you have been granted explicit permission by the account owner to provide access.
Credentials will be used strictly for service-related activities and handled in accordance with our internal security and confidentiality practices. We are not responsible for any pre-existing account conditions, third-party service limitations, or changes made outside the scope of authorized work.
Do not submit credentials unless you fully understand and agree to this authorization.
