This form is designed for one person to register the individuals from their facility. Each registrant will receive confirmation and instructions for accessing the sessions.

Upon completion of this form, the contact person will receive an invoice from Square our payment processor. If you wish to include the Contact person in the billing, please complete the fields below and check the box to include the contact person. This invoice is payable with a credit card. Please comment if a separate emailed invoice is required rather than that sent from Square.
