I (Name) understand that by participating in this workshop and subsequent workshops during January 1, 2026 -December 31, 2026 inclusive, I will be working with glass shards and I agree to exercise caution while handling these shards.

I agree to wear any and all personal safety equipment that is provided to me during the session (safety glasses, closed-toe shoes, Ziploc bags, etc.) As a participant in this workshop session, I release and agree to hold Lauren Richeson harmless from any and all liability and hereby assume all risk and responsibility and waive any claim or cause of action of any nature arising while participating in a creative glass session. I further agree to indemnify and hold harmless Lauren Richeson, and agents, employees or contractors, whether paid or volunteer, against any and all claims which may arise from any injury while participating in this program.

I understand photos may be taken where I am visible in a group setting and allow Lauren Richeson to use these for social media and website purposes.
