I hereby acknowledge that I have voluntarily enrolled myself and one or more dogs in dog training activities taught by Celtic K9 Academy Instructor Pamela Ladewig, Becki Hirschy or and Meriann Munn hereinafter "Instructor") which will be held at N2501 Wenham Road, Fort Atkinson, WI 53538, owned by Alistair A. and Meriann Munn (hereinafter "Munns") and operated by Celtic K9 Academy, LLC, (hereinafter "the Academy"). I have been advised and acknowledge that there are always inherent risks and hazards involved with and around dogs and in training facilities. I am aware that dogs can be inherently difficult to control and that not all dogs will be under control at all times during training activities. I have had full opportunity to discuss any and all concerns I have regarding the foregoing risks with Instructor, Munns, and the Academy and I have made inquiries and investigations to my satisfaction related to such risks, including, but not limited to an examination of the training area. I understand that this Informed Consent, Release of Liability, and Indemnification Agreement is intended to shift the risk of participating in dog training to me as the Participant/Owner/Guardian of the dog(s) participating in training unless any damages or injuries are the result of intentional or reckless acts of Instructor, Munns, the Academy, or any of their employees. I now state that I am voluntarily participating in the training activities with full knowledge of the potential risks and hazards.

Unless damages or injuries occur as the result of the intentional or reckless acts of Instructor, Munns, the Academy, of any of their employees, if any, I, for myself, my heirs, executors, administrators, legal representatives, successors and assigns, therefore, agree as follows:

1. I hereby assume the risks associated with partaking in such training.

2. I agree to waive any and all claims or actions against Instructor, Munns, and/or the Academy and fully release and discharge each of them from liability for any and all personal injuries to me, my dog(s), any minors in my charge, and any third parties, including death, for harm to any property caused directly or indirectly by any acts that might occur in conjunction with dog training classes, including any agility obstacles or to the facility or grounds on which the training facility sits, or to any other structure on the property on which the training is held even if caused by alleged negligence by Instructor, Munns, or the Academy, any visiting parties, or other persons.

3. I agree to assume sole and full responsibility for injury or damage caused by myself, children in my charge, or by the dog(s) I own or handle during participation in classes, events and activities.

I further agree to indemnify, defend, and hold harmless Instructor, Munns, and the Academy from any and all claims of damage, injury, loss, liability or expense, including legal costs and attorney fees, incurred by Instructor, Munns, or the Academy which are brought by third parties and result from or relate to my use of the

training facility and the training activities, the children in my charge, or the dog(s) I bring to the training activities, except for those resulting from the intentional or reckless acts of Instructor, Munns, or the Academy. I have been offered the opportunity to negotiate the terms and conditions of the release of liability and indemnification agreement; however, I choose to accept the terms and conditions of this agreement as they are, without negotiation. If any term or condition of this Agreement, or the application of this Agreement to any person or circumstance, shall be deemed invalid or unenforceable, the remainder of this Agreement, or the application of the term or condition to persons or circumstances other than those to which it is held invalid or unenforceable, shall not be affected thereby, and each term and condition shall be valid and enforceable to the fullest extent permitted by the law. I acknowledge that I am over eighteen (18) years of age and have full legal authority to sign this contract. I have carefully read this Informed Consent, Release of Liability, and Indemnification Agreement and fully understand the contests thereof
