MIKADO JUNIOR SUMMER OPERA CAMP
Participation Agreement & Waiver
By registering my child for Opera San Luis Obispo’s Mikado Junior Summer Opera Camp, I acknowledge and agree to the following:
Participation & Assumption of Risk
I am the parent or legal guardian of the above-named minor and am authorized to enter into this agreement on their behalf. I agree to release and hold harmless Opera San Luis Obispo, its directors, staff, volunteers, contractors, and the host venue on behalf of the minor and the minor’s heirs and representatives.
I understand that participation in opera camp activities, including movement, dance, staging, and performance, involves physical activity and carries a risk of injury. I voluntarily assume these risks on behalf of my child, except in cases of gross negligence or willful misconduct.
Medical Authorization
In the event of an emergency, I authorize Opera San Luis Obispo staff to obtain medical treatment for my child as deemed necessary. I understand that I am responsible for any medical expenses incurred. I have disclosed any medical conditions, allergies, or behavioral considerations that may affect my child’s participation. I confirm that the information provided in this registration form is accurate and complete.
Cancellation by Organization
Opera San Luis Obispo reserves the right to modify, reschedule, or cancel the program due to circumstances beyond its control. In such cases, tuition refunds or credits will be determined at the organization’s discretion.
Property / Personal Belongings
Opera San Luis Obispo is not responsible for lost, stolen, or damaged personal property brought to camp.
Photo & Media Release
I grant permission for Opera San Luis Obispo to photograph and/or record my child during camp activities and performances. I understand these images or recordings may be used for promotional, educational, or archival purposes without compensation.
Conduct & Dismissal Policy
I understand that respectful and cooperative behavior is expected at all times. Opera San Luis Obispo reserves the right to dismiss a participant for unsafe, disruptive, or inappropriate behavior. In such cases, tuition may be non-refundable.
Attendance & Commitment
I understand that this is a two-week intensive program and consistent attendance is required. Excessive absences may affect participation in the final performance and/or placement in the October production. Participation in camp does not guarantee a specific role in the October production. Casting and placement decisions are at the discretion of the artistic staff.
Tuition & Refund Policy
Enrollment is confirmed upon receipt of full payment. Tuition and registration fees are refundable until July 01, 2026. After July 13, 2026, tuition is non-refundable, except in cases of documented medical emergency. In the event of a documented emergency or illness during camp, a pro-rated refund will be applied.
Costumes / Property Damage
I understand that costumes, props, and materials must be handled with care. Families may be responsible for damage caused by intentional misuse or negligence.
Agreement
By signing below, I confirm that I have read and agree to the above policies and authorize my child’s participation in the Mikado Junior Summer Opera Camp.
