TEAM LIABILITY WAIVER, RELEASE OF CLAIMS & INDEMNIFICATION AGREEMENTRoseland Cease Fire Basketball TournamentThis Liability Waiver, Release of Claims, and Indemnification Agreement (“Agreement”) is executed by the undersigned team representative on behalf of themselves and all players listed on the official team roster (“Participants”).1. Assumption of RiskI acknowledge that participation in the Roseland Cease Fire Basketball Tournament involves inherent risks, including but not limited to physical injury, illness, permanent disability, or death, as well as property loss or damage. I voluntarily assume all such risks on behalf of myself and all Participants associated with my team.2. Release and Waiver of LiabilityIn consideration of being allowed to participate in the Roseland Cease Fire Basketball Tournament, I hereby release, waive, and forever discharge:Roseland Cease Fire ProjectPullman Community CenterThe City of Chicago, The City of Calumet ParkEvent sponsorsPartner churches, houses of worship, and faith-based organizationsVolunteers, staff, officials, referees, and event organizersAny other organizations or individuals involved in planning, managing, promoting, or hosting this event(collectively referred to as the “Released Parties”)from any and all claims, demands, causes of action, liabilities, damages, or losses arising out of or related to participation in the tournament, whether caused by negligence or otherwise, to the fullest extent permitted by law.3. Indemnification and Hold HarmlessI agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, actions, damages, costs, or expenses (including reasonable attorney’s fees) arising from:Participation by myself or any team memberAny injury or damage caused by a ParticipantAny violation of tournament rules or policies4. Medical AuthorizationI certify that all Participants are physically fit to participate and have not been advised otherwise by a medical professional. In the event of an emergency, I authorize the Released Parties to obtain medical treatment deemed necessary for any Participant. I understand that all medical expenses are the responsibility of the Participant or their guardian.5. Insurance AcknowledgmentI acknowledge that the Released Parties do not provide health or accident insurance for Participants. Each Participant is responsible for their own medical insurance coverage.6. Conduct & ComplianceI understand that Participants are expected to follow all tournament rules, facility policies, and codes of conduct. Failure to do so may result in removal from the event without refund or recourse.7. Media ReleaseI grant permission for Roseland Cease Fire and its partners to use photographs, video, and recordings of Participants for promotional, educational, and marketing purposes without compensation.8. Binding AgreementI understand that this Agreement is binding and enforceable. I affirm that I have read this document in its entirety, understand its terms, and sign it freely and voluntarily on behalf of myself and my team.
